Glaucoma Control Strategies in Sub-Saharan Africa: A Review of the Clinical and Health Economic Evidence

Abstract: While effective glaucoma control interventions exist, their widespread use and diffusion across SSA remain challenging principally due to low per capita income levels and high glaucoma treatment costs.

“…It starts at younger ages, and the progression is more pronounced. These factors and the limited diagnostic and therapeutic possibilities, especially in rural areas of Africa, are the reasons why glaucoma-related blindness is more common in Central Africa than in other parts of the world [29][30][31][32].…”

“…Trabeculectomy, the main surgical procedure for treating glaucoma, can effectively reduce IOP; however, the operation has a long learning curve, is offered in relatively few eye units across sub-Saharan Africa, can have clinically significant complications, and has a low uptake in some populations. 8 , 9 , 10 , 11 , 12 Selective laser trabeculoplasty (SLT) is a rapid outpatient procedure used to reduce IOP. SLT increases aqueous fluid outflow from the eye, which drains through the trabecular meshwork.…”

“…There is currently, for example, little understanding or consensus as to which glaucoma detection and treatment strategies are likely to be the most cost-effective in terms of the greatest number of sight years gained on a sustainable cost basis. Moreover, a recent literature review conducted by Smith et al (2018) determined that while a myriad of effective glaucoma control interventions exist in many urban centres throughout SSA, their widespread use and diffusion across SSA remains challenging principally due to their high cost and the lack of publicly funded eye health investments in glaucoma screening, detection and treatment alternatives [2]. This current situation across much of SSA exists despite the clear urgency of the problem at least as recognized by the global Vision 2020 coalition on blindness prevention and treatment [3].…”
