Glioblastoma Treatment Modalities besides Surgery

Abstract: Glioblastoma multiforme (GBM) is commonly known as the most aggressive primary CNS tumor in adults. The mean survival of it is 14 to 15 months, following the standard therapy from surgery, chemotherapy, to radiotherapy. Efforts in recent decades have brought many novel therapies to light, however, with limitations. In this paper, authors reviewed current treatments for GBM besides surgery. In the past decades, only radiotherapy, temozolomide (TMZ), and tumor treating field (TTF) were approved by FDA. Though pr… Show more

“…Gliomas continue to be the most common and devastating primary brain tumor. Despite multiple conventional therapies, including radiotherapy with adjuvant temozolomide chemotherapy after resection [ 1 , 2 ], patients with LGG (low grade glioma) have a median overall survival (OS) of 8–10 years, while patients with GBM (glioblastoma multi-form) have a dismal OS of less than 15 months [ 3 , 4 ]. Therefore, new therapeutic approaches are desperately needed.…”

“…For the treatment of glioma, standard post‐resection chemotherapy is given, which includes TMZ, although because of therapeutic resistance and tumour recurrence, there is often no decline in rate of mortality . TMZ is an agent for alkylating, that causes DNA methylation at the O 6 position of guanine causing incorrect pairing of thymine with O 6 ‐methylguanine, which initiates the mismatch repair system, causing a double‐strand break in the genome and subsequent cell cycle arrest and apoptosis .…”

“…The treatments for GBMs are surgery, followed by radiation and chemotherapy. But these treatments are largely ineffective and the median survival time for GBM is only 15 to 16 months [82]. Among the chemotherapy options, temozolomide (TMZ) improves the survival of patients with GBM, however, the development of resistance to GBM is very common and, thus, newer therapies are urgently required.…”
