The International Classification of Functioning, Disability and Health framework recognizes that an individual's functioning post-stroke reflects an interaction between their health condition and contextual factors encompassing personal and environmental factors. Personal factors significantly impact rehabilitation outcomes as they determine how an individual evaluates their situation and copes with their condition in daily life. A key personal factor is self-efficacy—an individual's belief in their capacity to achieve certain outcomes. Self-efficacy influences an individual's motivational state to execute behaviors necessary for achieving desired rehabilitation outcomes. Stroke rehabilitation practice and research now acknowledge self-efficacy and motivation as critical elements in post-stroke recovery, and increasing evidence highlights their contributions to motor (re)learning. Given the informative value of neuroimaging-based biomarkers in stroke, elucidating the neurological underpinnings of self-efficacy and motivation may optimize post-stroke recovery. In this review, we examine the role of self-efficacy and motivation in stroke rehabilitation and recovery, identify potential neural substrates underlying these factors from current neuroimaging literature, and discuss how leveraging these factors and their associated neural substrates has the potential to advance the field of stroke rehabilitation.

Spasticity Matters: A Call to Action Following an Acute Stroke

More than half of survivors of stroke experience some degree of motor impairment, and spasticity can develop within days of the initial event. Patients with post-stroke spasticity (PSS) report a lower quality of life than those without spasticity, and they require regular long-term follow-up and monitoring within the healthcare system. This symposium supported a non-promotional discussion regarding the prevalence, burden, consequences, and need for identification of PSS. The benefits of PSS identification within 3 months of stroke were discussed by a panel of key opinion leaders, including Ted Wein, Neurologist and Assistant Professor of Neurology and Neurosurgery at McGill University, Montréal, Quebec, Canada; Ganesh Bavikatte, Consultant and Clinical Lead in rehabilitation medicine at the Walton Centre, Liverpool, and Honorary Senior Clinical Lecturer at the University of Liverpool, UK; and Sean Savitz, Professor of Neurology and Physical Medicine and Rehabilitation, Frank M. Yatsu MD Chair in Neurology, and Director of the Institute for Stroke and Cerebrovascular Diseases, University of Texas Health Science Center at Houston (UTHealth), Texas, US. These key opinion leaders explained that early prediction of PSS could be improved by increased awareness of the associated risk factors and tools, such as the Post-Stroke Checklist (PSC), the Spasticity Screening Tool, and the PSS Referral Tool. Finally, potential barriers to the early identification of PSS were presented, alongside strategies to overcome these barriers.

Реабілітація є важливою стратегією охорони здоров'я і розглядається як комплекс заходів, спрямованих на допомогу особам з обмеженнями життєдіяльності для досягнення і підтримання оптимального функціонування у взаємодії з навколишнім середовищем. Визначення у пацієнтів реабілітаційного потенціалу та оцінка ефективності реабілітації є необхідною умовою для формування правильної реабілітаційної стратегії. Сучасним міжнародним інструментом, що визначає принципи прогнозування результатів реабілітації, оцінки реабілітаційного потенціалу та побудови реабілітаційних програм є Міжнародна класифікація функціонування, обмежень життєдіяльності та здоров'я (МКФ). Впровадження даних принципів дозволяє зробити прозорим процес реабілітації, що сприяє підвищенню якості медичної допомоги. Але крім оцінок за цими шкалами, необхідне використання певного методу визначення на їх основі індикативної інтегральної величини, що характеризує реабілітаційний потенціал пацієнта та його здатність до одужання. Мета дослідження полягала у виробленні науково-методичного підходу визначення ефективності реабілітаційних заходів на основі МКФ-профілей пацієнтів та програмної реалізації даного підходу у вигляді інформаційно-аналітичної системи. В роботі розглянуто сучасний стан способів оцінки реабілітаційного потенціалу та ефективності реабілітації на основі міжнародного стандарту МКФ. Проаналізовано апробовану методику оцінки ефективності реабілітації на основі обрахунку реабілітаційного інтегрального показника з точки зору можливості її вдосконалення та формалізації у вигляді певної математичної моделі. Програмна реалізація системи оцінки ефективності реабілітації створювалась за допомогою відповідних шаблонів проектування та на основі об'єктно-орієнтованого підходу. В результаті роботи вдосконалено методику оцінки ефективності реабілітаційних заходів та створено її математичну модель, що дозволяє оцінити комплексний результат реабілітації, використовуючи основні категорії МКФ. Розроблено програмну систему оцінки ефективності реабілітації на основі МКФ-профілей пацієнтів, яка дозволяє проводити комплексну аналітику процесу їх відновлення. Зокрема, система надає наступні можливості: визначити реабілітаційний потенціал пацієнта, що дозволяє оцінити його потенційну можливість до відновлення; встановити реабілітаційний інтегральний показник після курсу реабілітації, що дозволяє визначити рівень реалізації початкового реабілітаційного потенціалу та ступінь відновлення компенсаторних можливостей; проаналізувати динаміку зміни оцінок по МКФ-доменам та виявити найбільш проблемні напрямки. Отже використання створеної програмної системи оцінки ефективності реабілітації на основі МКФ-профілей пацієнтів дозволяє оцінити реабілітаційний потенціал у кожного конкретного пацієнта, визначить його потребу в реабілітаційних послугах, відстежить динаміку і оцінити ефективність відновного лікування та реабілітації в цілому.
