Graded Exercise Testing in a Pediatric Weight Management Center: The DeVos Protocol

Abstract: Obese youth seeking treatment at a stage 3 pediatric weight management center exhibit low VO2peak. The protocol outlined here should serve as a model for similar programs interested in the submaximal and peak responses to exercise in obese pediatric patients.

“…The cardiopulmonary exercise test (CPET) is a valuable tool to gain information on a patient's cardiac function and aerobic fitness and to determine how the heart, lungs, and peripheral musculature respond to the stress of exercise . In the cardiopulmonary exercise laboratory, the test is performed to assess functional capacity, aerobic fitness level, diagnose certain diseases, determine disease severity, and monitor the effects of treatment such as medication and exercise programs .…”

“…Children studied using a cycle ergometer for submaximal exercise testing had reduced ionotropic reserve compared with healthy matched controls . The graded exercise test is another valuable tool that evaluates a pediatric patient's cardiac function and aerobic fitness and determines how the heart responds to the extra demands of exercise . Our group is currently recruiting a cohort of long‐term HSCT survivors to determine if this modality may improve risk stratification for long‐term survivors.…”
