Granulocyte and monocyte adsorption apheresis as an effective treatment for Reiter disease

Abstract: Reiter disease (RD) is characterized by a triad of sterile arthritis, urethritis and conjunctivitis. The conditions occur concomitantly or sequentially, and are associated with mucocutaneous features such as circinate balanitis and stomatitis. Arthritis usually occurs in attacks followed by recovery, but it sometimes progresses to permanent damage of the affected joints. Because the symptoms of this disorder are attributable to activated neutrophils, we assessed the efficacy of granulocyte and monocyte adsorpt… Show more

“…Mice depleted of neutrophils are completely resistant to the inflammatory effects of autoreactive immunoglobulin from K/BxN mice, a model of RA that causes rapid, joint-specific inflammatory responses [41]. In humans, the efficacy of adsorptive granulocyte and monocyte apheresis has been reported for the treatment of many inflammatory conditions, such Reiter syndrome [42], Crohn's disease [43], and ulcerative colitis [44,45]. Although crude, these methods demonstrate the positive effect that reducing neutrophil numbers can have on disease pathology.…”

“…It features a column containing cellulose acetate (CA) beads . Our previous clinical studies on the efficacy of GMA for treating various skin diseases and related arthropathy attributable to activated granulocytes yielded excellent results in patients with pyoderma gangrenosum (PG), pustular psoriasis (PP), psoriatic arthritis (PsA), Behçet's disease (BD), adult Still's disease, leg ulcers associated with rheumatoid arthritis, cutaneous allergic vasculitis (CAV), Reiter's disease (RD), and systemic lupus erythematosus (SLE) skin rash .…”

“…DMARDs, such as sulfasalazine, are effective for peripheral manifestations, while most experts consider glucocorticoids use in ReA contraindicated, except for an occasional intra-articular injection (83). As ReA is attributable to activated neutrophils and it is histologically similar to pustular psoriasis in its prominent neutrophil infiltration, therapy with GMA may be useful for treating this disease, as reported by Yoshifuku et al (85). Particularly, the authors described a case of a 73-year-old man with scaly, coalescent erythematous macules with ulcers on penis and scrotum and a scaly erythematous plaque on his right hand.…”
