Grey-scale and colour Doppler ultrasound versus magnetic resonance imaging for the prenatal diagnosis of placenta accreta

Abstract: Placenta accreta can be successfully diagnosed by grey-scale and colour Doppler US. MRI would be more likely suggested for either posteriorly or laterally situated placenta previa in order to exclude placental invasion.

“…On screening the titles and abstracts, a further 162 were excluded as the reported outcomes were not relevant, leaving 58 studies which were obtained for full text review. An additional 38 articles were excluded after full review including letters (n=16), narrative reviews (n=10) commentaries (n=9), conference proceedings (n=2) and duplication of data in another publication (n=1), leaving 20 articles for the final analysis 19–38…”

“…Previously reported studies to diagnose MAP by 2D ultrasound have concluded a range between 76% -94% and 74% -99% for sensitivity and specificity respec- [20]. This wide range is probably attributed to the subjectivity of the 2D parameters and the variation in sample size.…”

“…6 Few studies compared the accuracy of the MRI and US in the antenatal diagnosis of placental accretism, and similar prediction results have been found for both methods. [7][8][9][10] Satija et al 8 concluded that the US remains the primary modality for the evaluation of placental accretism, and the MRI should be reserved for inconclusive cases. Rezk and Shawky 9 compared Doppler US and MRI in patients with placenta previa and uterine scarring.…”

Rezk and Shawky 9 compared Doppler US and MRI in patients with placenta previa and uterine scarring. Even though these authors did not make separate analyses for the placement of the placenta, they concluded that the MRI should be reserved to exclude accretion in cases of posterior or lateral placental location.…”
