Growth hormone therapy at the time of Covid-19 pandemic: adherence and drug supply issues

Abstract: Due to the outbreak of Covid-2019 pandemic, the organisation of many Hospitals in Italy, as well as all over the world, has changed dramatically. In our tertiary Endocrinology Unit, we are continuing to deliver urgent care to our patients, trying to grant assistance to chronic and frail subjects, as well. In this Letter, we provide a picture of GH adherence during Covid19 pandemic form one of the first and worst hit Italian region. We aim to share our experience in the management of both paediatric and adult p… Show more

“…Relatively surprisingly, data shows that most patients with GH deficiency of all ages, including children and adults, but also transition patients had overall good adherence to GH therapy during the COVID-19 pandemic (55). The authors' explanation of this better than expected adherence in the pandemic compared with previous data was linked to home confinement despite the disruption of clinical care for many patients.…”

“…We did not directly assess therapy adherence, but it has previously been described to be suboptimal in pediatric populations [ 24 ]. An increase in GH therapy adherence during the COVID-19 pandemic was reported by Giavoli et al, indicating that home confinement influenced the adherence rate positively [ 25 ]. We therefore speculate that the individuals with PWS received a more continuous GH administration due to more flexible family routines and fewer travels during the lockdown.…”

“…Findings of the potential for increased adherence (as we observed for patients taking tacrolimus and buprenorphine/naloxone) are not without precedent either. A single-center study of patients receiving growth hormone therapy did not identify Covid-related increases in non-adherence (though they did highlight the need for increased caregiver diligence due to supply issues) [20], and increased adherence post-Covid has been reported in a study of 7578 asthma and COPD patients using an inhaler [21]. However, this could be related to patient vigilance in the setting of a pulmonary pandemic.…”
