Guidance for Researchers Developing and Conducting Clinical Trials in Practice-based Research Networks (PBRNs)

Abstract: Background There is increased interest nationally in multicenter clinical trials to answer questions about clinical effectiveness, comparative effectiveness, and safety in real-world community settings. Primary care practice-based research networks (PBRNs), comprising community- and/or academically affiliated practices committed to improving medical care for a range of health problems, offer ideal settings for these trials, especially pragmatic clinical trials. However, many researchers are not familiar with w… Show more

“…25 Second, practice-based research networks of clinical practice groups, often driven by academic investigators, have contributed to the understanding of genetic and other biomarkers regarding treatment responses. [26][27][28] Third, commercial laboratories have collaborated with care delivery systems to conduct studies assessing clinical validity and utility of new tests. 29 Fourth, the Electronic Medical Records and Genomics (eMERGE) Network of nine distinct systems use electronic medical records as a tool for genomic research.…”

“…Emerging evidence documents the overall experience 614 and outcomes 1516 of participating in practice-based research (PBR). Practicing clinicians, 611 academic researchers, 12 and experts in PBR 14 have described their experiences in or made recommendations about implementing PBR.…”

“…Practicing clinicians, 611 academic researchers, 12 and experts in PBR 14 have described their experiences in or made recommendations about implementing PBR. To assess direct experiences, Yawn et al 13 used semi-structured telephone interviews of 48 project nurses and physicians who participated in a PBR project around postpartum depression conducted in small to medium-sized family medicine clinics in which six major themes emerged: (1) systematic approaches, (2) teamwork and communication, (3) spread to other conditions, (4) professional self-worth and recognition, (5) staff “expand” into new roles and (6) research literacy.…”
