Background/Aim: Morbus Hansen (MH) is a chronic infectious disease caused by Mycobacterium leprae. The disease has an average incubation period of 3-5 years. Morbus Hansen (MH) has various clinical, microbiological and histopathological features. In the following, a new case of MH is reported with a clinical manifestation of Borderline Tuberculoid (BT) type MH. The patient was treated with the MDT MB regimen for 3 months and had clinical improvement. Case Report: A 23-year-old boy with complaints of sudden feeling of heat, tingling and cramping from the tip of his right foot to the sole of his right foot. He also complaining about black lesions on both hands, right elbow, face, chest and back. The diagnosis of gout, fungus and dermatitis can be ruled out after microscopic examination of AFB on the right, left ear lobe, and foot lesions showed negative results and histopathological examination results showed Morbus Hansen type BT. The risk factor for contracting Morbus Hansen is the patient has lived in a dormitory for 2.5 years and it is known that there are people who suffer from a similar disease in that environment. The patient was treated using a multibacillary regimen for 3 months and the lesions improved and the numbness decreased. Treatment plan during the case for 12-18 months. This case is unique due to the clinical manifestation has not significant characteristic for MH, AFB is not found from ear lobe smear. MH established after skin biopsy done. Conclusion: It was reported that one MH case with TB for 3 months had clinical improvement, with plans to continue treatment for 12-18 months. Steroid-refractory hand eczema: treatment experience

Hand eczema is a common multi-etiological disease manifested by evolutionary polymorphism of rashes. Hand eczema develops against the background of altered reactivity and genetic predisposition under the adverse effects of endogenous and exogenous factors. Hand eczema has a chronic course and is characterized by a wide range of clinical manifestations. The prevalence of eczematous lesions of the hands among the adult population is 1–2%, and among all skin diseases – 30–40%. The disease has an extremely negative impact on the quality of life, ability to work, career prospects and the social status of a person. The long course of eczematous lesions of the hands can be complicated by a secondary infection: bacterial and mycotic flora. Therefore, choosing the right and effective treatment for hand eczema is a particularly difficult task for a dermatologist. In recent years, a multicomponent drug based on gentamicin sulfate, dexpanthenol, mometasone furoate and econazole nitrate has been widely used for the treatment of hand eczema due to its high efficacy, tolerability, and safety. The use of this multicomponent drug in monotherapy for hand eczema is effective, leads to clinical remission and restoration of the quality of life of patients and is not accompanied by side effects and complications. This review focuses on the epidemiology, clinical features, and treatment options for hand eczema. Clinical experience with the use of a multicomponent drug in patients with hand eczema complicated by infection is also given.

Background Chronic hand dermatitis (CHD) is difficult to treat and has high individual and societal burdens. Phototherapy and oral alitretinoin are safe monotherapies for CHD, but their combination has not been assessed. Objective To assess the effectiveness and safety of low dose oral alitretinoin combined with phototherapy versus high dose oral alitretinoin for CHD refractory to topical corticosteroids. Methods This retrospective study of adult patients with CHD refractory to topical corticosteroid therapy compared low dose oral alitretinoin (10 mg three times weekly) combined with narrowband ultraviolet B therapy (three times weekly; LDA-UVB) to high dose oral alitretinoin (30 mg daily; HDA) for 16 weeks. Outcomes were improvement in disease severity measured by the Physician’s Global Assessment and quality of life measured with the Dermatology Life Quality Index. Results The mean age of the study population ( n = 64) was 41.25 years and 57.8% were male. Both cohorts experienced improvements in disease severity and quality of life after 16 weeks, however, significantly more participants who received LDA-UVB ( n = 21/33, 63.6%) achieved “clear” or “almost clear” assessments compared to those who received HDA ( n = 12/31, 38.7%; P < .05). Adverse effects were significantly more prevalent in the HDA group ( P < .0001) and included headache, elevated cholesterol, and dry lips. Conclusions The combination of low dose oral alitretinoin with narrowband-UVB therapy was more effective and had fewer adverse effects compared to high dose oral alitretinoin for participants with CHD refractory to topical corticosteroid therapy.
