Gunshot Wounds of Abdomen: Evaluation of Stable Patients with Triple-Contrast Helical CT

Abstract: In stable patients with gunshot wounds to the abdomen in whom there is no indication for immediate surgery, triple-contrast helical CT can help reduce the number of cases of unnecessary or nontherapeutic laparotomy (negative laparotomy) and can help identify patients with injuries that may be safely treated without surgery.

“…Computed tomography of the abdomen, besides being one of the protocol items, has a number of advantages and is essential for this kind of approach, there being evidence level II recommendations 16 . With the advancement of technology, multidetector CT has a sensitivity of 91% to 97%, a specificity of 96% to 98% and an accuracy of 96% to 98% in detecting intra-abdominal injuries in patients with GSW abdominal injuries [17][18][19] . Among the benefits of NOM are the decreasing frequencies of unnecessary laparotomies and their complications, length of hospital stays and overall treatment costs.…”

“…A delayed or recurrent accumulation of simple density pleural fluid may also suggest thoracic duct injury, which is associated with chylous thoracostomy tube drainage [59]. VATS may be performed for evacuation of retained hemothorax that fails multiple attempts at thoracostomy drainage, for empyema with tenacious adhesions, or for late-phase fibrothorax [2,57].…”

“…The majority of torso gunshot wound and some stab wound victims are hemodynamically unstable or have overt peritoneal signs and undergo exploration without pre-operative CT [1][2][3][4]. Multiple injuries are seen in most torso gunshot wounds, and patients in shock will often require initial damage control surgery, during which hematomas are explored, bleeding vessels are ligated, and splenectomies and single nephrectomies are performed in unsalvageable cases.…”
