Gunshot Wounds to the Face: Emergency Interventions and Outcomes

Abstract: UAC and UBC are required frequently after GSWF and are associated with intraoral involvement and injuries beyond the face. Simple methods, such as orotracheal intubation and packing, are typically sufficient for successful management. About half of the patients need further surgery, with infrequent morbidity.

“…Os ferimentos por projétil de arma de fogo causam efeitos nocivos e se apresentam como um desafio para os cirurgiões de reconstrução por conta das consequências dramáticas que comprometem a estética e a funcionalidade da face (Orthopoulos et al, 2013;Maurin et al, 2015). A avaliação inicial é de fundamental importância para o planejamento do tratamento dos tecidos e guiará o cirurgião na tomada de decisão sobre a intervenção imediata ou tardia das lesões (Miloro, 2016).…”

“…Stylianos [17 ]reported the effectiveness of abdominal packing for severe hemorrhage in refractory abdominal and retroperitoneal sites and in hepatic hemorrhage in pediatric patients. Other situations in which packing has been used to effectively tamponade severe hemorrhage in adults include thoracic hemorrhage [18,19], facial trauma [20], pelvic fractures [21,22,23], abdominal trauma [24], liver trauma [25], maxillofacial trauma [26], and postpartum hemorrhage [27]. …”

“…In addition, there is a risk of associated injuries to the cervical spine, base of the skull, carotid arteries and the brain [2]. Orotracheal intubation and packing was reported to be sufficient primary management with about half of the patients needing further surgical repair [3]. Emergency intubation in civilian penetrating gunshot injuries was reported in 7 to 13% of cases, the need for an early surgical airway was between 11% and 35% [1, 2, 3, 4].…”

Emergency intubation in civilian penetrating gunshot injuries was reported in 7 to 13% of cases, the need for an early surgical airway was between 11% and 35% [1, 2, 3, 4]. Impaired airways were more frequently observed after high-velocity injuries and in cases with injuries to the tongue, floor of the mouth, and midline or bilateral facial skeletal bones [4].…”
