Gynaecological uses of a new class of steroids: the selective progesterone receptor modulators

Abstract: Selective progesterone receptor modulators (SPRM) represent a new class of synthetic steroids, which can interact with the progesterone receptor (PR) and can exert agonist, antagonist or mixed effects on various progesterone target tissues in vivo. This review evaluates the actual and potential usefulness of SPRMs in gynaecology.

“…4 Intrauterine delivery of progestin is an effective way to administer local treatment and bypass the systemic side effects. In addition to providing highly effective contraception, intrauterine progestin delivery is safe and effective in the management of menorrhagia, dysmenorrhoea, uterine myomata and endometrial proliferation.…”

“…The selective progesterone receptor modulators (SPRMs) are progesterone receptor ligands that could bind with progesterone receptors and exert antagonistic, agonistic or mixed agonistic-antagonistic effects depending on the cellular context of the target tissues [9], [10]. Among several hundred, nearly a dozen of SPRMs have been evaluated to any significant extent.…”

“…46,47 Other SPRMs have both agonistic and antagonistic properties at the level of the progesterone receptors. 48 SPRMs inhibit cell proliferation and trigger apoptosis of the cultured leiomyoma cells. 49 Asoprisnil is a SPRM and reduces leiomyoma size without hypoestrogenism and bone loss.…”
