Introduction Skin quality is an important component of human attractiveness. To date, there are no standardized criteria for good skin quality. To establish a consensus for good skin quality parameters and measurement and treatment options, a virtual skin quality advisory board consisting of a global panel of highly experienced aesthetic dermatologists/aesthetic physicians was convened. Methods A total of 10 dermatologists/aesthetic physicians served on the advisory board. A modified version of the Delphi method was used to arrive at consensus. Members accessed an online platform to review statements on skin quality criteria from their peers, including treatment and measurement options, and voted to indicate whether they agreed or disagreed. Statements that did not have agreement were modified and the members voted again. Consensus was defined as: strong consensus = greater than 95% agreement; consensus = 75% to 95% agreement; majority consent = 50% to 75% agreement; no consensus = less than 50% agreement. Results There was strong consensus that good skin quality is defined as healthy, youthful in appearance (appearing younger than a person’s chronological age), undamaged skin and that skin quality can be described across all ethnicities by four emergent perceptual categories (EPCs): skin tone evenness, skin surface evenness, skin firmness, and skin glow. The EPCs can be affected by multiple tissue layers (ie, skin surface quality can stem from and be impacted by deep structures or tissues). This means that topical approaches may not be sufficient. Instead, improving skin quality EPCs can require a multilayer treatment strategy. Conclusion This global advisory board established strong consensus that skin quality can be described by four EPCs, which can help clinicians determine the appropriate treatment option(s) and the tissue or skin layer(s) to address. Skin quality is important to human health and wellbeing and patients’ perception for the need for aesthetic treatment.

Optical factors affecting hair appearance are reviewed based on hair structures from macroscopic to microscopic viewpoints. Hair appearance is the result of optical events, such as reflection, refraction, scattering, and absorption. The effects of hair structures on such optical events are summarized and structural conditions for hair appearance are considered. Hair structures are classified into the following: the alignment of multiple hair fibers, the cross-sectional shape of the hair fiber, and the microstructures of hair fiber (cuticle, cortex, and medulla). The alignment of multiple hair fibers is easily affected by the existence of meandering fibers and their alignment along hair length becomes less-synchronized. The less-synchronized orientation of multiple fibers causes the broadening of the apparent reflection and luster-less dull impression. The cross-sectional shape of hair fiber affects light reflection behavior. Hair fibers with elliptical cross-section show glittering colored light based on total reflection in the hair. The scaly structures of cuticles at the surface of hair are often uplifted and cause light scattering, and then affect hair luster. The porous structure of the cortex and medulla in hair fiber can cause light scattering and affect hair luster and color. The above phenomena suggest that important factors for hair appearance are the alignment of multiple hair fibers, appropriate cross-sectional shape, ordered scaly structure, and pore-less internal structure.

The purpose of this paper is to discuss the most important factors affecting perceived age. Aging is an unavoidable and irreversible process, but perceived age is a modifiable psychosocial factor. Our exterior has become one of the determinants of our social position, the key to success in our professional and personal lives. Today, we see people through their appearance, we judge after the first impression. With the help of aesthetic medicine, we can change the perceived age, but we cannot stop aging. This article discusses factors affecting the perceived age, such as hair color, skin color, general appearance and environmental factors. Many cannot be avoided, but we can make changes in life to look younger and more well-groomed. Indeed, we can change our habits for the benefit of the whole body, not only for the skin.
