Background The number of online services in health care is increasing rapidly in developed countries. Users are expected to take a more skilled and active role in taking care of their health and prevention of ill health. This induces risks that users (especially those who need the services the most) will drop out of digital services, resulting in a digital divide or exclusion. To ensure wide and equal use of online services, all users must experience them as beneficial. Objective This study aimed to examine associations of (1) demographics (age, gender, and degree of urbanization), (2) self-rated health, (3) socioeconomic position (education, experienced financial hardship, labor market position, and living alone), (4) social participation (voting, satisfaction with relationships, and keeping in touch with friends and family members), and (5) access, skills, and extent of use of information and communication technologies (ICT) with perceived benefits of online health care and social welfare services. Associations were examined separately for perceived health, economic, and collaboration benefits. Methods We used a large random sample representative of the Finnish population including 4495 (56.77% women) respondents aged between 20 and 97 years. Analyses of covariance were used to examine the associations of independent variables with perceived benefits. Results Access to online services, ICT skills, and extent of use were associated with all examined benefits of online services. ICT skills seemed to be the most important factor. Poor self-rated health was also consistently associated with lower levels of perceived benefits. Similarly, those who were keeping in touch with their friends and relatives at least once a week perceived online services more often beneficial in all the examined dimensions. Those who had experienced financial hardship perceived fewer health and economic benefits than others. Those who were satisfied with their relationships reported higher levels of health and collaboration benefits compared with their counterparts. Also age, education, and degree of urbanization had some statistically significant associations with benefits but they seemed to be at least partly explained by differences in access, skills, and extent of use of online services. Conclusions According to our results, providing health care services online has the potential to reinforce existing social and health inequalities. Our findings suggest that access to online services, skills to use them, and extent of use play crucial roles in perceiving them as beneficial. Moreover, there is a risk of digital exclusion among those who are socioeconomically disadvantaged, in poor health, or socially isolated. In times when health and social services are increasingly offered online, this digital divide may predispose people with high needs for services to exclusion from them.

Objectives A nationwide patient portal (My Kanta) for viewing electronic prescriptions and health data has been phased in since 2010 in Finland. This study aimed to study how commonly Finnish pharmacy customers use My Kanta, the factors related to My Kanta use, the main reasons for non‐use and how non‐users would like to monitor their medication and health information. Methods A survey was conducted among adult pharmacy customers purchasing prescription medicines for themselves or for their child <18 years. Questionnaires (N = 2866) were distributed from 18 pharmacies across Finland. Open‐ended questions were analysed qualitatively. Quantitative analyses included frequencies, Chi‐square tests, Fisher's exact tests, t‐tests and logistic regression analysis. Key findings In total, 994 (34.7%) questionnaires were included. Most (82.5%) adult pharmacy customers used My Kanta. Use of the service was associated with use of the internet to search for health‐related information (OR: 8.82, 95% CI: 4.65‒16.74), active internet use (OR: 7.30, 95 %: CI 3.54‒15.08), living in Northern (OR: 4.35, 95% CI: 1.75‒10.82) or Eastern (OR: 3.25, 95% CI: 1.41‒7.48) parts of Finland, and the increasing number of currently used regular prescription medicines (OR: 1.16, 95% CI: 1.01‒1.34). The main reasons for non‐use were lack of need and tools. Non‐users reported physician/health centres and pharmacies as their preferred sources of medication and health information. Conclusions Most Finnish pharmacy customers use the My Kanta nationwide patient portal. The strongest predictors for use are factors related to internet use. Some pharmacy customers do not use My Kanta despite having the necessary means. The main reason for non‐use is a lack of need. Customers unable to use My Kanta want to monitor their medication and health information via healthcare professionals.

Vahva tunnistautuminen mahdollistaa henkilöllisyyden todentamisen digipalveluissa. Digipalvelut ovat arjen palveluita esimerkiksi laskunmaksu verkkopankissa tai terveystietojen tarkastelu Omakannassa. Selvitämme tutkimuksessa onko eri väestöryhmissä eroja internetin ja vahvan tunnistautumisen käyttömahdollisuudessa. Aineisto muodostettiin väestökyselyistä: 1) Ulkomailla syntyneiden hyvinvointitutkimus (FinMonik, 2018−2019), joka toteutettiin 18 eri kielellä ja vastausprosentiksi muodostui 53 (N=12 877). 2) Koko väestön Kansallinen terveys-, hyvinvointi- ja palvelututkimus (FinSote, 2017−2018), jonka vastausprosentiksi muodostui 45 (N=26 422). Rajasimme vastukset työikäisiin (20−64 vuotiaisiin) ja analyyseissä käytettiin 6 083 vastausta FinMonikista ja 11 029 FinSotesta. Tulosmuuttujille laskettiin ikävakioidut osuudet ja luottamusvälit. Itseilmoitetulle vahvan tunnistautumisen käyttömahdollisuudelle laskettiin vetosuhteet (odds ratio) logistisella regressiomallinnuksella. Estimaattien eroja maaryhmien ja koko väestön välillä tarkasteltiin F-testiin perustuvilla p-arvoilla. Työikäisessä väestössä lähes kaikki (98 %) raportoivat, että heillä oli mahdollisuus käyttää internetiä, mutta ulkomaalaistaustaisen väestössä osuus oli matalampi (92 %, p < 0,001). Itseilmoitetun vahvan tunnistautumisen käyttömahdollisuus oli yleisempää koko väestössä (98 %) kuin ulkomaalaistaustaisessa väestössä (88 %, p < 0,001). Ulkomaalaistaustaisessa väestössä Lähi-itä ja Pohjois-Afrikka -taustaiset raportoivat muita ryhmiä harvemmin, että heillä oli mahdollisuus vahvaan tunnistautumiseen. Molemmissa kyselyissä nuorilla itseilmoitettu vahvan tunnistautumisen käyttömahdollisuus oli yleisempää verrattuna vanhempiin henkilöihin ja opiskelijat ilmoittivat työssä olevia harvemmin vahvan tunnistautumisen käyttömahdollisuudesta. Itseilmoitetussa vahvassa tunnistautumisessa oli väestöryhmittäisiä eroja. Pohdittavaksi jää eritoten palkkatyön merkitys digiosallisuuden kasvattajana. Palkkatyö mahdollistaa varat laitteiden hankintaan, käyttömaksuihin sekä usein kerryttää digiosaamista. Digitaalisia palveluita tarjottaessa huomiota tulisi kiinnittää väestöryhmiin, joissa vahvan tunnistautumisen käyttömahdollisuus on muuta väestöä heikompaa. Digitaalisten palveluiden kehittäminen vaatii rinnalleen ohjausta, jossa huomioidaan neuvonnantarpeet myös vahvan sähköisen tunnistautumisen osalta.
