BACKGROUND: Halo nevus is a benign condition characterized by symmetrical oval-shaped and well-defined hypopigmented patch surrounding melanocytic lesions. Their sizes varies from a few millimeters to centimeters. A “wait and see” approach is usually preferred in most cases of halo nevus. However, the condition can cause severe psychological impact and impair patient’s quality of life, especially when it emerges on prominent areas such as the face. Surgical option can be utilized for removal of melanocytic lesions and accelerate regression of the halo. CASE PRESENTATION: We report a case of a 25-year-old woman with white patches around two moles on her face who was diagnosed as halo nevus. Surgical excision was performed and a 12-week follow-up showed reduction in diameter of the halo nevus. CONCLUSION: Large halo nevus can cause significant esthetic burden that may affect patient’s quality of life. A more invasive approach such as excision of underlying nevus may be used to achieve more rapid results.

Halo nevus (HN) is benign skin condition with a central melanocytic nevus, surrounded by an area or halo of depigmentation. It is the result of immunological response of the body toward the nevus, which destroys the melanocytes in surrounding skin, leading to the depigmented halo. An increased frequency of HN in patients with vitiligo is observed. It is more commonly seen in children or young adults of either sex, particularly on the trunk, less commonly on the face, neck, and limbs. We present a rare case of HN which was present on the lower eyelid associated with poliosis, diagnosed with dermatoscopy.

Halo nevi (leukoderma acquisitum centrifugum, Sutton's nevus, leukopigmentary nevus, perinevoid vitiligo or perinevoid leukoderma) are melanocytic nevi surrounded by a halo of hypopigmentation or depigmentation. They are significantly more common in children and young people, with a peak incidence at the age of 15, without gender or racial predilection. The usual clinical course of a halo nevus involves gradual involution and subsequent complete regression of a centrally placed nevus that leaves behind an area of depigmentation. Typical halo nevi do not require treatment. Exceptionally, halo nevi develop from dysplastic nevi and malignant neoplasms of the skin (halo phenomenon). The paper presents a 12-year-old boy with changes in nevi on his back. During the last summer, a pale band developed around two nevi. The nevi became lighter and shrank. One of them has completely disappeared. Apart from basal cell carcinoma in the father, the presence of other atypical nevi, melanoma, vitiligo and autoimmune diseases in the personal and family anamnesis was not established. A typical halo nevus is diagnosed by clinical and dermatological examination and regular dermatological monitoring is advised. Halo nevi mostly develop from an acquired melanocytic nevus, but can be seen in dysplastic nevi, basal cell carcinoma, and melanoma. A thorough medical history, careful clinical and dermatological examination make it possible to identify atypical halo nevi that require a biopsy.
