Hand-held cardiac ultrasound screening performed by family doctors with remote expert support interpretation

Abstract: HCU performed at the point of care by FDs with remote expert support interpretation using a web-based system is feasible, rapid and useful for detecting significant echocardiographic abnormalities and reducing the number of unnecessary echocardiographic studies.

“…This is similar to previous data, in which a 68% decrease of indications of conventional echocardiography was achieved through handheld echocardiography in PC 17. These data suggest task sharing of echocardiography in the PC system may both be cost-saving and optimise resource utilisation by decreasing unnecessary referrals and prioritising high-risk patients.…”

“…In the PC setting, similar approaches have proven to be feasible. Evangelista et al reported a small proportion of handheld images (8.8%) as suboptimal for interpretation, a considerable decrease of referrals for conventional echocardiography and an increase of discharges, suggesting this strategy may be cost-saving 17. Our study protocol was even less expensive, as the images were also acquired by non-physicians.…”

“…47,57,58 Limited cardiac examinations 20,52 also had a higher quality estimate than more extensive examinations. 27,39,67 Several articles 17,63,64 have reported an improvement in diagnostic accuracy of ultrasonography with training, but we found no overall association between the amount of training and diagnostic accuracy (Supplemental Appendix 5).…”

“…Evangelista et al [16] have claimed that handheld ultrasound performed at the point of care by family doctors, with remote expert support interpretation using a web-based system, is feasible, rapid, and useful for detect-ing significant echocardiographic abnormalities and decreasing the number of unnecessary echocardiographic tests. a good knowledge of ultrasound scanning techniques among family physicians could help cut the cost of medical care, since family physicians' ability to interpret the results correctly would most probably reduce the number of improper referrals to specialists [3].…”
