Handlebar hernia with intra-abdominal extraluminal air presenting as a novel form of traumatic abdominal wall hernia: Report of a case

Abstract: An 18-year-old male was admitted to our Emergency Department with a traumatic abdominal wall hernia (TAWH) of the left lower quadrant (LLQ) after suffering hypogastric blunt injury and urogenital lacerations in a motorcycle accident. Upright chest X-ray showed a small amount of right infradiaphragmatic free air, and a computed tomographic (CT) scan demonstrated an abdominal wall hernia. “…Many a times herniation is seen at anatomically weak points due to blowout near iliac crest( lumbar),inguinal region or lateral to rectus muscle away from site of primary impact. 7 Tension of abdominal musculature between pelvic bones increases potential for disruption in right lower abdomen lateral to rectus muscle through oblique and transverse muscle after blunt abdominal trauma.…”

“…While ultrasonography provides adequate assessment of the presence or absence of free fluid following blunt abdominal trauma, it is of limited usefulness in evaluating the abdominal wall. If traumatic hernia is suspected, CT scan should be implemented preferentially in the diagnostic workup [14,[19][20][21].…”

“…In fact, there are only a few reports of associated injuries occurring. 3,4,7 According to the past reports, the diagnosis of TAWH is based mainly on physical examinations, US, and CT. In many cases, a tender bulging or swelling in the injured area was recognized as soon as the patient was examined.…”
