Abstract— Leprosy is a chronic infectious disease considered one of the oldest diseases in history, which mainly affects the skin and peripheral nerves. In Brazil, there is a need to evaluate and characterize the number of leprosy cases, as well as their distribution throughout the national territory. Thus, the present study aimed to carry out an analysis of the spatial distribution and trace the epidemiological profile of leprosy cases in Brazil between the years 2012 and 2021. For this, data available in the Sistema de Informação de Agravos de Notificação (SINAN) were collected and analyzed. The variables evaluated were: Gender, Age group (years), race, education, Type of Output, Classification of leprosy and clinical form. The most affected individuals were men (56.92%), aged between 40 and 59 years (37.68%), mixed race/color (57.89%) and elementary school (48.66%). The states of Maranhão, Mato Grosso and Pará had the highest numbers of leprosy cases in Brazil. Fortunately, most patients have progressed to a cure. Based on the clinical form, there was a predominance of the Dimorphic form, followed by the Virchowian form. In turn, in relation to operational classification, most cases were classified as multibacillary. In conclusion, we emphasize that future studies are needed to assess the impact of the COVID-19 pandemic on SINAN notifications and confirm whether the reduction in leprosy cases in Brazil is the result of better public policies or due to underreporting. We also present valuable information from states and strategic groups for health campaigns aimed at combating leprosy.

BACKGROUND Leprosy is a chronic infectious disease that affects the peripheral nerves, leading to neurological damage and physical disability. This study analysed the temporal trend in the rate of new leprosy cases with grade 2 physical disability in Brazil and identified spatial clusters of risk for physical disability. METHODS This is an ecological study of all new cases of leprosy with grade 2 physical disability (G2D) at diagnosis between 2001 and 2022 in Brazil, macro-regions, states and municipalities. Data were extracted from the National System of Notifiable Diseases and the Brazilian Institute of Geography and Statistics. Sociodemographic and clinical variables such as age, sex, race/colour, education and clinical form of the disease were analysed, as well as the rate of new cases of leprosy with grade 2 physical disability at the time of diagnosis per million inhabitants. Data analysis was divided into three stages: 1- Descriptive analysis of the socio-demographic and clinical profile; 2- Time series analysis: In this stage, the joint-point regression model was used; 3- Spatial analysis, using global and local Moran statistics. RESULTS There were 50,466 new cases of leprosy with G2D during the period studied, 97.4% aged > 14 years, 70.2% male, 45.7% brown, 87.6% with low education, 90.2% multibacillary and 47.5% dimorphic clinical form. There was a downward trend in the detection of leprosy with physical disability grade 2, with an average annual reduction of 2.8% (13.6/1 million in 2001 and 9.1/1 million in 2022). The southeast, south and north regions showed a downward trend. A total of 14 states and 4 capitals showed a decrease in the rate, the most significant being Roraima (-11.0%/year). Tocantins was the only state with an upward trend (3.2%/year). 635 (14.1%) municipalities were in the high-high quadrant. These municipalities belong to 12 Brazilian states and accounted for 30% (n = 15,139) of all cases recorded in the country over the 22 years analysed. CONCLUSIONS These results suggest an overall decrease in the detection of leprosy with physical disability in Brazil and highlight geographical disparities in the occurrence of leprosy disability. Interventions to promote early diagnosis should be targeted at areas with the highest rate of new cases with grade 2 disability.

Introdução: O Brasil é a segunda nação com mais novos casos de Hanseníase no mundo. Em comparação ao restante do país, a região Sudeste é referência na prevenção e tratamento da doença. O presente estudo tem o objetivo de interseccionar as informações de saúde sobre Hanseníase quanto ao modo de entrada no sistema de saúde dos casos diagnósticos por ano, escolaridade e quantidade de lesões cutâneas dos pacientes a fim de verificar as relações do contexto socioeconômico na persistência da doença na macrorregião do Centro-Oeste do estado de São Paulo. Métodos: Estudo epidemiológico ecológico descritivo, retrospectivo, com dados secundários sobre os casos da Hanseníase por ano diagnóstico no período de 2012 a 2021 no território da Rede Regional de Atenção à Saúde (RRAS) 10, do Departamento Regional de Saúde de Marília-SP (DRS IX), a qual engloba 62 municípios da macrorregião do Centro-Oeste paulista. Resultados: Quanto ao modo de entrada dos casos ao sistema de saúde, há predomínio de novos casos. A maior parte são de indivíduos com 4ª série do EF incompleta ou completa, enquanto a menor parcela dessa população possui Educação Superior incompleta ou completa. Há predomínio de condições mais graves com mais de cinco lesões cutâneas. Discussão: Sob análise da variável escolaridade como representante da condição socioeconômica, embora a Hanseníase possa atingir qualquer indivíduo, verifica-se maior prevalência em grupos carentes socioeconomicamente. O predomínio de pacientes multibacilares pode estar relacionado também à escolaridade, uma vez que a baixa compreensão da população sobre a Hanseníase faz com que a busca por atendimento médico aconteça somente quando a doença já está em estágios mais avançados. Conclusão: Há coexistência entre circunstâncias sociais, inferidas pela escolaridade, e prevalência dos casos de Hanseníase no território e período estudados. Reforça-se a necessidade de ações educativas contínuas sobre a doença à população.
