Hashimoto thyroiditis: an evidence-based guide: etiology, diagnosis and treatment

Abstract: Hashimoto thyroiditis (HT) is a common autoimmune disorder, affecting women 7-10 times more often than men, that develops because of genetic susceptibility, X-chromosome inactivation patterns, modulated by environmental factors as well as microbiome composition, and leading to an imbalance in the self-tolerance mechanisms. The consequential thyroid infiltration by the lymphocytes, potentiated by antibody-mediated autoimmune response through the antibodies against thyroid peroxidase (TPOAbs), leads to a destruc… Show more

“…Hashimoto's thyroiditis characterized by lymphocytic infiltration, which may finally lead to the progressive loss of thyroid tissue [1]. Some studies prove that HT is the outcome of the united action of genetic susceptibility and environmental factors, but the precise route is still not established [13].…”

“…Hashimoto's thyroiditis is known to be an essential endocrine disease that affects the population and may lead to hypothyroidism. This disease is one of the most commonly spread autoimmune diseases [1]. Hypothyroid patients frequently experience features of depression which is prevalent among other symptoms in hypothyroidism [2].…”

“…Cells of Hashimoto's thyroiditis appear immune reactive to high molecular weight keratin or p63. Elevated kappa/lambda ratio occurs due to enhanced production of kappa light chains secreted by plasma cells [4,5] . Admixed infiltrate of T lymphocytes and B lymphocytes appears polyclonal [4,5].…”

“…Several genes have been involved in HT pathogenesis, including genes of the immune response (coded in the Human Leukocyte Antigen (HLA) complex) and thyroid function. Other immunoregulatory genes are involved in the development of HT, including the single nucleotide polymorphisms (SNPs) in cytotoxic T lymphocyte-associated antigen 4 (CTLA-4), protein tyrosine phosphatase non-receptor type 22 (PTPN22), and CD40 [ 2 , 4 , 6 , 7 , 18 , 19 , 20 ].…”
