Healing of Granulomatous Skin Changes in Ataxia‐Telangiectasia After Treatment with Intravenous Immunoglobulin and Topical Mometasone 0.1% Ointment

Abstract: Ataxia-telangiectasia (AT) is a rare autosomal recessive disorder characterized by faulty DNA damage repair. The disease affects multiple systems and is noted to be particularly difficult to diagnose in children because of the wide spectrum of clinical presentations. We present an unusual case of a child in whom the primary cutaneous manifestation of AT was noninfectious cutaneous caseating granulomas. A 3-year-old girl presented to the emergency department with ataxia, poor growth, and multiple ulcerated plaq… Show more

“…Nonetheless, high potency topical corticosteroids and cyclosporine A can be used for healing of small superficial ulceration . In addition, more considerable non‐infectious granulomas may respond to fumaric acid esters (FAE), systemic inhibitors of tumor necrosis factor (TNF‐alpha), direct injection of steroids into the site of the granulomatous ulceration, and combination therapy such as topical steroids with concurrent immunoglobulin replacement therapy …”

“…More extensive granulomas may respond to combination therapy (e.g. topical steroids plus IV gamma globulin therapy) [152], systemic inhibitors of tumor necrosis factor (TNF-alpha) [153] or direct injection of steroids into the site of the granulomatous lesions [154]. …”

“…To date, there are no known treatments that are highly effective. In most cases, the inflammation has progressed relentlessly unless hematopoietic stem cell transplantation (HSCT) has been performed with resolution of the immunodeficiency, although one patient has been reported to have responded to topical steroid treatment [7]. Rubella virus is insensitive to type I interferons and no other antiviral therapy is of proven benefit.…”
