Health Care Delivery Restructuring and Productivity Change: Assessing the Veterans Integrated Service Networks (VISNs) Using the Malmquist Approach

Abstract: This study evaluates the productivity changes for the Veterans Integrated Service Networks (VISNs) that the Veterans Health Administration (VHA) created, comparing performance in 1994 with that in 2004. This represents periods before and after the VHA in 1995 reconfigured provider units into 21 regionalized delivery systems and engaged in other important system innovations. Productivity is measured using the Malmquist Index approach (a longitudinal version of the data envelopment analysis [DEA]). Results indic… Show more

“…Variable return to scale (VRS) normally forms a convex hull of two intersecting plans which envelop the data points more tightly as compare to the CRS DEA which makes a conical hull and thus provide technical efficiency scores which are greater than or equal to those which are obtained using CRS model [13]. In table 4 the last column shows the nature of the scales whether that is increasing return to scale (IRS) or decreasing returns to scale (DRS).…”

“…Ozcan and Luke Ozcan and Luke (2011) used an extension of DEA, called the Malmquist technique, to examine the impact of a major restructuring that the Veterans Health Administration (VHA) implemented in 1995 on productivity change. The VHA decentralized decision making from VHA headquarters to regions and integrated services and assets regionally to create 21 regional providers called Veterans Integrated Service Networks (VISNs).…”

“…For example, the VA has grown from operating 54 hospitals in 1930 to now operating 171 medical centers, 350 outpatient clinics, 126 nursing home care units, and 35 domiciliary care units for veterans. 8 Congress supported the reorganization of the VA health care system, creating comprehensive health care facilities, which led to improved quality of care. 7 In 1995, the VA initiated organizational restructuring designed to improve quality and increase efficiency.…”
