Health care disparities among octogenarians and nonagenarians with stage III lung cancer

Abstract: In this NCDB analysis, approximately 62.7% of patients aged ≥80 years with stage III NSCLC received no cancer-directed care. Black race and living in a lower educated census tract were associated with not receiving cancer-directed care. OS was found to be improved in patients receiving CRT. “…For the majority of cancers studied, older age was associated with nonadherence to guideline-concordant treatment, in keeping with data from other studies. 7 , 39 , 40 , 41 For example, Wockel et al 7 showed that older age was associated with nonadherence to guidelines in patients with breast cancer. Although we found that generally older, sicker patients did not undergo guideline-concordant treatment, it is not necessarily due to their inability to tolerate it, given that the nonguideline-concordant treatment was generally more aggressive.…”

“…This benefit is possibly related to access to healthcare, known to be important in clinical outcomes for many cancer subsites. [32][33][34][35][36] This study has several strengths and limitations. The strengths include the largest number of resected nonmetastatic head and neck ACC of any study to date, all treated in the modern era.…”

“…This association persists even after controlling for known factors that influence treatment tolerability and response: sex, comorbidities, performance status and histology [ 5 , 19 21 ]. Studies from lung cancer registries in other Western countries, including the USA and Germany, have also demonstrated under-treatment of elderly patients with lung cancer [ 22 , 23 ]. In addition to replicating these findings in an Australian setting, our study identifies high-risk groups within the elderly population who can be targeted for future intervention: those with advanced-stage lung cancer and those not discussed at multidisciplinary meetings.…”
