Health Literacy Associated With Blood Pressure but not Other Cardiovascular Disease Risk Factors Among Dialysis Patients

Abstract: Limited health literacy is common in individuals on dialysis. Individuals with adequate health literacy skills had DBP readings that were on average 13.8 mm Hg lower and MAP that was 14.8 mm Hg lower than those with inadequate health literacy. Prospective studies to elucidate if improvements in health literacy skills will lead to improvement in BP control are needed.

“…The prevalence of limited health literacy in individuals with end-stage renal disease who are on maintenance dialysis has been found to be between 16% and 32% [9, 10, 23, 24, 25, 26]. However, studies assessing health literacy in predialysis CKD are scarce, with only one other published study reporting on the prevalence of limited health literacy in this population [26].…”

“…In terms of outcomes, Green et al [26] reported that limited health literacy was independently associated with an increased incidence of missed dialysis treatments, emergency department visits, and dialysis related hospitalizations. Another study of 72 patients on hemodialysis reported that people with limited health literacy had worse blood pressure control than those with adequate health literacy [27] . Moreover, limited health literacy has been associated with an increased risk of death in hemodialysis patients [24] .…”

“…47,48 In hemodialysis, the largest study of 480 participants found 32% with limited literacy, 49 similar to other studies, although these range from 16% to 50%. 48,50-53 Limited health literacy is consistently associated with fewer years of education, non-White race, and male gender. 49,52 The disparities observed in these vulnerable populations may be explained in part to differences in health literacy, as seen with diabetes and numeracy.…”

“…49 Limited health literacy has been found in other samples to be associated with higher average blood pressure, 53 more missed dialysis treatments, and more frequent use of emergency room services. 55 In addition, those with limited literacy were more likely to have ESKD-related hospitalizations with an adjusted incidence rate ratio of 1.55 (1.03-2.34; P = .04).…”
