Health-Related Quality of Life in Patients With Autosomal Dominant Polycystic Kidney Disease and CKD Stages 1-4: A Cross-sectional Study

Abstract: Background In people with early autosomal dominant polycystic kidney disease (ADPKD) average total kidney volume (TKV) is three times normal and increases by an average of 5% per year despite seemingly normal glomerular filtration rate (GFR). We hypothesized that increased TKV would be a source of morbidity and diminished quality of life that would be worse in subjects with more advanced disease. Study Design Cross-sectional. Setting & Participants 1043 subjects with ADPKD, hypertension and a baseline esti… Show more

“…4,5 Deterioration of renal function is usually proportional to the increase in kidney size, and many patients with ADPKD and renal dysfunction have massive kidneys. They develop various symptoms related to abdominal distension that impair their quality of life, 6,7 and there can also be an adverse influence on the prognosis. Methods for reducing the size of enlarged kidneys include aspiration and injection of sclerosant into renal cysts, 8,9 surgical and laparoscopic cyst fenestration, 8,[10][11][12] and surgical and laparoscopic nephrectomy.…”

“…Comparing pain intensity in women and men did not show a significant difference and the results were the same with FPS-R and NRS scales. However, other studies indicated that the pain intensity in women was higher than men (23,24). In a study done in California, older women, with a low income, who were insufficiently active during their leisure time, had higher prevalence of chronic pain (25).…”

“…8 Pain is not related to kidney size early in the disease process (eGFR>60mL/min/1.73m 2 ) unless the kidneys are extremely large with a height adjusted kidney volume greater than 1,000 mL/m. 5 Pain presentation based on cyst size can be variable as some patients with smaller cysts can experience severe pain, while other patients with larger cysts remain pain free. 6 Renal associated pain in ADPKD can present in various ways, making it difficult to clinically confirm its etiology.…”

“…5 Compression on the stomach and duodenum can result in decreased appetite and abdominal fullness, leading to a risk of malnutrition. 5,6 Chronic Liver Pain in ADPKD Hepatic cysts in ADPKD can be identified in up to 94% of patients ages 35-46 years through magnetic resonance imaging. The cyst volume and incidence increases with age as the disease progresses.…”
