Health Technology Assessment of Public Health Interventions: A Synthesis of Methodological Guidance

Abstract: The heterogeneity in approaches indicates that there is currently no consensus on methods to deal with the challenges of the PHI evaluations. A possible explanation for this may be that the methods are not sufficiently developed, and advantages and disadvantages of a certain method in relation to the research question (e.g., broad/focused) have not yet been sufficiently evaluated.

“…The inadequacy of HTA from an overall PH perspective is also reported in literature (12,(25)(26)(27). One of the underlying reasons probably lays in the multi-dimensional character of PH, a field which is increasingly affected by a full range of extra-clinical circumstances (28,29), including demographic transitions (30) and modifiable risk factors of disease development associated with unhealthy lifestyle habits in the general population (31)(32)(33)(34)(35).…”

“…The key issue with building the evidence base for initiatives such as social prescribing as well as PH interventions in general lies in the fact that applying the golden standard of evidence (i.e., double-blinded randomized controlled trials) might not be adapted. Pragmatic trials, as a more naturalistic approach, might provide a better picture of effectiveness, while evidence summaries may include a realist synthesis as opposed to standard systematic review approaches (12,(18)(19)(20). This being said, it should be kept in mind that good evidence does not necessarily lead to good policy (21).…”

“…Authors of primary and secondary research find context challenging to operationalise, research and evaluate 23. Primary studies are criticised for failing to characterise context,17 yet many guidance documents recommend that contextual factors are examined within systematic reviews and syntheses 24. Review teams face particular challenges resulting from (1) the lack of contextual detail in included studies and (2) unsophisticated approaches to handling context within existing review tools.…”

“…Although randomized controlled trials are considered the standard method for the evaluation of health interventions (Cochrane, 1989), they are not often available in the field of public health as they are usually difficult to conduct (Petticrew et al, 2012). Mathes et al (2017), reviewed the existing HTA guidelines for this type of technology, and concluded that methods to evaluate public health interventions are not sufficiently developed or have not been adequately evaluated.…”
