Hearing preservation cochlear implantation in children: The HEARRING Group consensus and practice guide

Abstract: The HEARRING group concludes that irrespective of the degree of residual hearing present, the concepts of hearing and structure preservation should be applied in every child undergoing cochlear implantation and that HPCI is a safe and reliable treatment option. Early detection and multidisciplinary assessment are key to the identification of children with symptomatic PD, these children should undergo HPCI as early as possible.

“…Electrical stimulation of the auditory nerve via CIs probably reestablishes crucial central functions for fast auditory processing by initiating activity in high-SR fibers. This must be concluded from the success in achieving nearly normal speech and language development in congenitally deaf children (Rajan et al, 2018;Albalawi et al, 2019). This partial implementation of fast auditory processing through CIs may also be reflected in the observed shortened response-onset latencies observed after cochlear electrode implantation in cats (Tillein et al, 2016).…”

“…Despite growing experience and documentation, there is still no consensus about optimal electrode trajectory pathways and array designs for structure/hearing preservation. There is a strong recommendation that CI surgery in children should be performed with the principle of structure/hearing preservation, regardless of the degree of preoperative hearing (Rajan et al, 2018).…”

“…The criteria for exclusion from the study were as follows: comorbidity: hypertension, diabetes or cancer, and contraindications for steroid therapy (taking drugs that may weaken or intensify the effects of steroids: anticoagulants, antidepressants, immunosuppressants), and hypersensitivity to the drug or its components. The inclusion criteria are in line with the international HEARRING Group consensus on hearing protection for cochlear implant users [20].…”
