Hearing preservation in patients undergoing vestibular schwannoma surgery: comparison of middle fossa and retrosigmoid approaches

Abstract: The results of this study have demonstrated a more favorable hearing outcome for patients with intracanalicular tumors and tumors extending up to 1 cm into the CPA that were removed via the MF when compared with the RS approach.

“…The adherence of the tumor shell to the nerve itself and, tumor size and form usually determine the intensity of mechanical manipulation during dissection of tumor. Our 6 month follow up results for facial nerve preservation was similar and comparable to other reports [4,7,33,39,41,[43][44][45]. Therefore, the use of the laser in proximity of the nerve seems to be safe enough and is recommended especially when having a direct view of the facial nerve such as in the RS approach.…”

“…47 Satar et al 81 believe that facial nerve dysfunction following the middle cranial fossa approach is transient and that the long-term outcome is the same as with the other approaches. This approach may be poorly tolerated by the elderly, as extradural dissection of the adherent dura may be difficult.…”

“…In experienced hands, complete tumor removal is achieved in 80 to 99% of the patients. [1][2][3][4][5] The major goal has become preservation of neural functions, in particular of facial nerve function and hearing, which are the most important predictors of the quality of life.…”
