Helical CT of aorta after endoluminal stent-graft therapy: value of biphasic acquisition.

Abstract: OBJECTIVE. Wereport ourprospective studyevaluating biphasic helical CT of theaorta after endoluminal stern-graft placement. SUBJECTSAND METHODS. Biphasic helical CT scans in95 patients withabdominaland thoracic aortic aneurysms who had undergone endoluminal stent-graft placement were re viewed. After a test bolus injection of 15 ml of contrast media at a rate of 3.5 mi/sec for the measurement of the optimal start delay, the aorta was scanned using the following parameters: collimation of 5 mm, table speed of 7… Show more

“…Currently, the classification most recommended by the vascular surgeon's societies is the one developed by White et al, that was modified in recent years, dividing endoleaks into five types (19)(20)(21) . This classification is based on the origin of the blood flow into the aneurysmal sac and has direct implication on the treatment required for each case.…”

“…For this reason, multiphase CT angiography has been recommended: a typical protocol includes images before the administration of contrast medium, images after the administration of contrast in the arterial phase and images in the delayed phase. Pre-contrast images can be useful in the differentiation of calcifications in the endoleak aneurysmal sac, thus reducing the number of inconclusive studies (20,28) .…”

“…The control examination after intervention should include an unenhanced acquisition and two contrast-enhanced CT data sets (arterial and delayed phase). Some authors recommend delayed phase imaging because endoleaks show variable flow rates and are, therefore, not necessarily detected during the early contrast-enhanced phase [16,20].…”

“…3 Currently, the gold standard follow-up modality after EVAR is computed tomography angiography (CTA). 4 The benefits of CTA over duplex ultrasound scanning (DUS) are that it is highly reproducible, less influenced by body habitus, and offers faster image acquisition. There are however important limitations, including repeated radiation exposure, contrast-related complications (eg, allergy and renal nephropathy), and the high cost of serial examination.…”
