Hemoptysis as the presenting manifestation of bronchiectasis-associated hospitalization in Korea

Abstract: Background Patients with bronchiectasis commonly experience disease exacerbations, which cause significant morbidity and mortality. However, data regarding the clinical features of bronchiectasis patients hospitalized with hemoptysis are scarce. Methods We retrospectively collected the data of patients with bronchiectasis-associated hospitalization at a tertiary referral center in Korea, and classified them into the hemoptysis and infective exacerbation (IE) groups. The… Show more

“…Our study demonstrated that bronchiectasis patients with hemoptysis had less severe bronchiectasis (the Reiff score for all but two patients was six or lower). This is in accordance with a study by Seo and colleagues who also showed that bronchiectasis with hemoptysis had a lower bronchiectasis severity and short-term mortality [31]. The explanation for those results and ours-with lower severity scores-may be that the hemoptysis phenotype is not associated with severe bronchiectasis but with specific etiology and radiological findings.…”

“…Patients who had hemoptysis were less frequent in the lowest sex-specific PMA quartile group compared with other groups. Our previous study reported that bronchiectasis patients hospitalized with hemoptysis had a lower bronchiectasis severity and 30-day mortality than those with infective exacerbation [35]. In this study, the lowest sex-specific PMA quartile group was associated with higher severity and mortality, which may explain the low percentage of patients presenting with hemoptysis.…”

“…Hemoptysis is frequently associated with bronchiectasis, occurring in 22–25% of afflicted patients [ 36 , 37 ]. A markedly higher proportion of bronchiectasis patients admitted due to hemoptysis have a history of pulmonary TB, with an odds ratio (OR) of 1.66 (95% confidence interval [CI], 1.03–2.69; p = 0.038) [ 41 ]. The increased prevalence of hemoptysis in post-TB bronchiectasis isn’t yet fully understood.…”

“…The increased prevalence of hemoptysis in post-TB bronchiectasis isn’t yet fully understood. However, this group often exhibits marked reductions in lung function, as demonstrated by diminished forced expiratory volume in one second (FEV 1 ) and greater evidence of bronchial artery hypertrophy—a major hemoptysis contributor [ 41 44 ]. It’s conceivable that severe symptoms, such as consistent coughing and sputum production, could intensify pressures on nearby blood vessels.…”
