Hepatic falciform artery: angiographic observations and significance

Abstract: The angiographic incidence of the HFA is more common than previously reported. The delayed and persistent opacification of the HFA on hepatic angiograms caused by its slow blood flow is considered the key to its identification.

“…Since late-2009, our institution has routinely performed SPECT/CT after liver-lung shunt scintigraphy. Our incidence of falciform ligament radiotracer activity detected on SPECT/ CT is 10% (5 out of 51 patients), in keeping with reported angiographic incidence [8][9][10][11][12][13]. However, this is likely an under-representation as some of our patients do not undergo whole-liver injection of Tc-99m-MAA.…”

“…The angiographic incidence of a patent hepatic falciform artery ranges from 2 to 24.5% [8][9][10][11][12][13]. Since late-2009, our institution has routinely performed SPECT/CT after liver-lung shunt scintigraphy.…”

“…The frequency reported by Song was higher, which might be caused by larger volume and quicker injection of contrast agent, or by selective angiography of the left hepatic artery. It was reported that chemoembolization of this artery would cause reaction of the umbilical skin [15,17] , which was not observed in our study. We believe that the hepatic falciform artery is a residual artery from the embryo that supplies blood from liver to the umbilical tissues.…”

“…While high incidence of HFA (67.2%) was reported on postmortem anatomic dissections, the reported incidences of HFA on angiography were low (ranging from 2% to 24.5%) [7-9]. One of the reasons for these discrepancies seems to be the velocity of arterial flow.…”

“…One of the reasons for these discrepancies seems to be the velocity of arterial flow. The flow was often too slow to be regularly detected during the early arterial phase on angiography [9]. The similarity of the HFA route with those of other arteries might be another reason.…”
