Herpes simplex virus type 2 meningitis without genital lesions: An immunoblot study

Abstract: Two sexually active female patients presented with acute meningitis. The CSF abnormalities were severe and persistent. In spite of the absence of genital lesions, serological studies revealed a primary infection by herpes simplex virus type 2. An immunoblot study revealed intrathecal synthesis of anti-herpes antibodies early in the course of the disease.

“…The antigens used in both methods are however not specific for HSV type 1. The immunoblotting technique had already detected anti-HSV intrathecal responses in cases of menigitis and encephalitis due to HSV type 2 (Boucquey et al, 1990;Monteyne et al, 1996). The same is true for ELISAs (Ashley et al, 1991).…”

“…Detection of serum and CSF anti-HSV and anti-VZV antibodies was carried out by immunoaffinity blotting [Boucquey et al, 1990;Sindic et al 1994]. CSF and sera diluted to the same IgG concentration were analysed by isoelectric focusing using agarose gels.…”

“…[ 10 , 11 ] Infection with HSV-2 beyond the neonatal period in an otherwise healthy individual is almost always associated with a self-limited form of meningitis. [ 28 ] In contrast, HSV-1 typically causes rapidly progressive fatal necrotizing encephalitis [ 29 ] with a mortality rate of up to 70% in the absence of therapy. [ 12 , 30 ] In our study, all patients were fully recovered and discharged.…”

“…The intrathecal demonstration of oligoclonal IgG bands from MS patients by electrophoretic profiling can be used for diagnosis. However, the simultaneous increase of IgGs against different viruses may rather indicate a generalized inflammatory reponse, because infections enhance only monospecific IgGs directed against the pathogen (Boucquey et al 1990; Sindic et al 1990). In fact, binding of these antibodies to viral target structures in the CNS has not been demonstrated.…”
