Herpes Zoster Recurrences More Frequent Than Previously Reported

Abstract: H erpes zoster (HZ), or shingles, is usually considered a once-in-a-lifetime experience, with HZ recurrences thought to be limited to immunocompromised individuals. The plausibility, actual occurrence, and frequency of recurrent HZ have been debated. [1][2][3][4][5] Studies of HZ recurrence often focus on special populations with hematologic malignancies or exposure to chemotherapeutic or immunotoxic agents like arsenic, [6][7][8][9] or they are population studies with small numbers of cases or short follow-up… Show more

“…HZ normally occurs only once, but second and third HZ episodes are possible and do happen occasionally. The frequency of repeated HZ increases with time after the first HZ illness in persons with a healthy immune system from 1.7% (95% CI 1.0-2.3) after 2 years to 3.2% (95% CI 2.3-4.2) after 4 years, then to 4.4% (95% CI 3.3-5.4) after 6 years and 5.7% (95% CI 4.4-6.9) after 8 years [9]. VZV replication can be reactivated, leading to HZ disease, in particular in cases of waning T-cell-specific immunity (e. g. with advancing age or on immunosuppressive therapy, see also risk factors), but also spontaneously in immunocompetent persons and in young adults, children, and adolescents [10].…”

“…As with varicella, the magnitude of VZV-specific T cell immunity in shingles is inversely correlated with disease severity and the risk of serious consequences such as post-herpetic neuralgia [16]. A single episode of herpes zoster results in a rapid marked T cell response and typically protects individuals from repeated zoster episodes [16], although recurrence is observed in around 6% of immunocompetent herpes zoster patients in both Japan and the USA [18,19]. …”

“…40 However, it is possible to have more than one episode of HZ but the recurrence rate is low and accordingly to the literature ranges between 1.5 and 12.5%; the most likely rate is in the range of 4-5%. 41 The most important factor for recurrence is the duration of the pain during the acute episode.…”
