Hidradenitis suppurativa treated with finasteride

Abstract: Background: Hidradenitis suppurativa (HS) is a distressing condition for which no satisfactory treatment is available. Studies on hormonal mechanisms responsible for HS point towards altered end-organ sensitivity, probably related to the enzyme 5a reductase that converts testosterone to dihydrotestosterone. Finasteride, an inhibitor of type II 5a reductase, has been reported to be effective in recalcitrant HS. Aim: To study the effectiveness and tolerability of finasteride in patients with HS in a preliminary … Show more

“…To date, the use of finasteride has been reported in 12 HS patients-eight adults and four children. Seven adult patients showed a good response on 5 mg finasteride daily after a treatment period of 2-12 weeks [127,128]. Two patients showed recurrence 1 month after discontinuation.…”

“…10 Other case series have also shown therapeutic benefit for antiandrogens in some patients with HS. [11][12][13] Finasteride is a competitive inhibitor of the type II 5␣-reductase enzyme, which converts testosterone to dihydrotestosterone, resulting in decreased serum dihydrotestosterone levels. Type II 5␣-reductase is found in hair follicles, whereas type I 5␣-reductase is present in both hair follicles and apocrine glands.…”

“…12 To our knowledge, no long-term safety studies for finasteride have been performed in the pediatric population. Studies in adults examining the effects of finasteride on sperm count and motility showed no significant effect after 1 year of treatment, 23 but there have been a few case reports of reversible negative effects on spermatogenesis and 2 reports of an association with infertility.…”
