BackgroundThe incidence of gestational diabetes mellitus (GDM) is increasing worldwide. GDM patients have a significantly higher rate of cesarean section and postpartum hemorrhage, suggesting changes in uterine contractility. TWIK-1-related potassium channel (TREK1) expressed in the pregnant uterus and its role in uterine contraction. In this study, we examined the expression of HIF-1α and TREK1 proteins in GDM uterine and investigated whether high glucose levels are involved in the regulation of human uterine smooth muscle cells (HUSMCs) contraction through TREK1, and verified the role of HIF-1α in this process.MethodsCompared the uterine contractility between GDM and normal patients undergoing elective lower segment cesarean section. The HUSMCs were divided into normal glucose group, high glucose group, normal glucose with CoCl2 group, CoCl2 with echinomycin/L-Methionine group, and high glucose with echinomycin/L-Methionine group; Compare the cell contractility of each group. Compared the expression of hypoxia-inducible factor-1α (HIF-1α) and TREK1 protein in each group.ResultsThe contractility of human uterine strips induced by both KCl and oxytocin was significantly lower in patients with GDM compared with that in normal individuals, with increased TREK1 and HIF-1α protein expression. The contractility of cultured HUSMCs was significantly decreased under high glucose levels, which was consistent with increased expression of HIF-1α and TREK1 proteins. The contractility of HUSMCs was decreased when hypoxia was induced by CoCl2 and increased when hypoxia was inhibited by echinomycin. The TREK1 inhibitor L-methionine also recovered the decreased contractility of HUSMCs under high glucose levels or hypoxia.DiscussionThe high glucose levels decreased the contractility of the myometrium, and increased expression of HIF-1a and TREK1 proteins play a role in changes in uterus contractility.

Editorial on the Research TopicThe mechanisms of parturition and preterm birth Parturition is a complex process involving changes at the hormonal, physiological, morphological, immune, and metabolic levels. Worldwide, preterm birth (PTB), defined as parturition occurring between 20 and 37 weeks of gestation, is a major determinant of neonatal mortality and morbidity, often with long-term adverse effects on the health of both mother and child.In recent decades, the incidence of PTB has continued to rise in most countries. According to the World Health Organization, the rate of PTB across 184 countries is reported to be 5~18% of all births. Therefore, it is urgent to find ways to prevent preterm labor, and for this, a better understanding of the mechanisms behind it and those associated with parturition is necessary.In this Research Topic we bring together important papers that carefully disclose the mechanisms of parturition and preterm birth and provide potential technologies and biomarkers to understand and predict them. This will definitely help us find more effective clinical interventions to prevent preterm birth and improve the outcome of parturition in the future.Innovation of instrumentation and methods remains the major step in an understanding of the processes involved in birth. The use of electromyography (EMG) for parturition and preterm birth prediction has been found to have higher specificity and sensitivity (1, 2). Recently, H Liu's group graded uterine EMG activities during labor and suggested Low-grade EMG indicated high possibility of oxytocin treatment in the followup labor process and also longer first stage and total labor time (3). In this Research Topic, Romero-Morales et al. enhanced classification of preterm-term birth using continuous wavelet transform and entropy-based methods of electrohysterogram (EHG) signals, and Qi et al. reported two fetal membrane indicators derived from Magnetic Resonance Imaging that can predict premature rupture of membranes (PROM) and preterm premature rupture of membranes (PPROM). Olmos-Ramıŕez et al. have also found that uterine activity modified responses of the fetal autonomic nervous system at preterm active labor using a novel time-frequency analysis of fetal heart rate variability (FHRV). Background The transition of the myometrium from a quiescent to a contractile state during labour is known to involve inflammation, which is characterized by the infiltration of immune cells and the secretion of cytokines. However, the specific cellular mechanisms underlying inflammation in the myometrium during human parturition are not yet fully understood. Methods Through the analysis of transcriptomics, proteomics, and cytokine arrays, the inflammation in the human myometrium during labour was revealed. By performing single‐cell RNA sequencing (scRNA‐seq) and spatiotemporal transcriptomic (ST) analyses on human myometrium in term in labour (TIL) and term in non‐labour (TNL), we established a comprehensive landscape of immune cells, their transcriptional characteristics, distribution, function and intercellular communications during labour. Histological staining, flow cytometry, and western blotting were applied to validate some results from scRNA‐seq and ST. Results Our analysis identified immune cell types, including monocytes, neutrophils, T cells, natural killer (NK) cells and B cells, present in the myometrium. TIL myometrium had a higher proportion of monocytes and neutrophils than TNL myometrium. Furthermore, the scRNA‐seq analysis showed an increase in M1 macrophages in TIL myometrium. CXCL8 expression was mainly observed in neutrophils and increased in TIL myometrium. CCL3 and CCL4 were principally expressed in M2 macrophages and neutrophils‐6, and decreased during labour; XCL1 and XCL2 were specifically expressed in NK cells, and decreased during labour. Analysis of cytokine receptor expression revealed an increase in IL1R2, which primarily expressed in neutrophils. Finally, we visualized the spatial proximity of representative cytokines, contraction‐associated genes, and corresponding receptors in ST to demonstrate their location within the myometrium. Conclusions Our analysis comprehensively revealed changes in immune cells, cytokines, and cytokine receptors during labour. It provided a valuable resource to detect and characterize inflammatory changes, yielding insights into the immune mechanisms underlying labour.
