High-grade glial tumors, survival & prognostic factors; 5-year experience from Ankara Oncology Hospital

Abstract

OBJECTIVESThe aim of this study is to evaluate high-grade glioma patients treated with adjuvant radiotherapy in terms of demographics, overall survival and prognostic factors. METHODSRecords of 128 (81 male, 47 female) adult patients with high-grade glioma treated with radiotherapy between January 2003-December 2007 were retrospectively reviewed. Of these cohorts, 98% had pathologically confirmed subtypes and among those, 46% (n=58) underwent total exision, 34% (n=43) underwent subtotal exision and 14% (n=18) biopsied. Median age was 46 for grade 3 tumors and was 53 for grade 4. KPS was ≤70 for 40%. Sixteen GBM patients received concurrent, 14 received adjuvant TMZ. RESULTSFor a median folllow-up of 12 (1-80) months, 1 and 3-year overall survival for the entire group were 44.5% and 15.3% respectively. Median survival was 21 months for grade 3 and 9 months for grade 4. Age ≤50 (p=0.002), biopsy versus total and subtotal resection (p<0.001), preoperative KPS >70 (p<0.001) were statistically significant good prognosticators. Concurrent and/or adjuvant use of temozolamide had positive impact on survival of grade 4-GBM patients (p=0.02 and p<0.001). CONCLUSIONIn our series, KPS, resection type and age were found to be the most significant prognostic factors. Despite the use in small number of GBM patients, concurrent and/or adjuvant TMZ, positively affected survival.Key words: Brain tumor; radiotherapy; prognostic factors. AMAÇYüksek gradlı glial tümör tanısıyla adjuvan radyoterapi alan hastaların demografik özelliklerinin, genel sağkalımlarının ve prognoza etki eden faktörlerinin incelenmesi. GEREÇ VE YÖNTEMOcak 2003-Aralık 2007 tarihleri arasında yüksek gradlı glial tümör tanısıyla radyoterapi uygulanan 128 (81 erkek, 47 kadın) erişkin hasta verileri geriye dönük olarak incelendi. Patolojik tanı almış hastalar %98 oranındaydı ve bu hastaların %46'sına (n=58) total eksizyon, %34'üne (n=43) subtotal eksizyon ve %14'üne (n=18) biyopsi yapıldı. Grad 3 hastalarda ortanca yaş 46, grad 4 hastalarda ortanca yaş 53 olarak bulundu. KPS ≤70 olan hastalar %40 (n=50) oranındaydı. Temozolamid, GBM tanılı hastaların 16'sında eş zamanlı, 14'ünde adjuvan olarak verildi. BULGULAROrtanca takip süresi 12 ay (1-80) olan tüm hasta grubunda bir ve üç yıllık genel sağkalım sırasıyla %44.5 ve %15.3'tü. Grad 3 hastalarda ortanca sağkalım 21 ay, grad 4 hastalarda ortanca sağkalım dokuz aydı. Yaşın 50'nin altında olması (p=0.002), biyopsiye oranla total ve subtotal rezeksiyon yapılmış olması (p<0.001) ve cerrahi öncesi KPS'nin 70'in üstünde olması (p<0.001) iyi prognostik özellik taşımaktaydı. Temozolamidin eş zamanlı ve/veya adjuvan verilmesi grad 4-GBM tanılı hastalarda sağkalımı önemli oranda pozitif etkilemişti (p=0.02 ve p<0.001). SONUÇÇalışmamızda yaş, rezeksiyon genişliği ve KPS sağkalıma etki eden faktörler olarak bulunmuştur. Eş zamanlı ve/veya adjuvan temozolamid az sayıda GBM tanılı hastada kullanılmış olsa da bu hastalarda sağkalım üzerine olumlu katkı sağladı.
