High-Resolution MRI of Carotid Plaque With a Neurovascular Coil and Contrast-Enhanced MR Angiography: One-Stop Shopping for the Comprehensive Assessment of Carotid Atherosclerosis

Abstract: In patients with carotid atherosclerosis, ongoing CE-MRA with a neurovascular coil for the simultaneous detection of unstable plaques is feasible. Our MR protocol accurately identifies the major features of vulnerable plaque.

“…Ulceration has often been characterized as a depression of >1 mm in width and depth 36,37 ; however, its real definition is loss of endothelial lining. Noninvasive imaging evaluation is more adequately used in ACAD reserving catheter-based angiogram for endovascular interventions and it is rarely for diagnostic purposes 36,[38][39][40][41] . The sensitivity and specificity of the DU on detecting ulceration have improved over the years 36,38,39 .…”

“…Methemoglobin in IPH serves as an endogenous contrast agent leading to a shortened longitudinal relaxation constant T1 and hyperintensity on T1-weighted images. This hyperintense property of IPH has been noted on general-purpose T1-weighted images, including spin-echo and TOF acquisitions [16, 32]. The mask image from contrast-enhanced MRA acquisitions can also be used to detect IPH and has led to greater detection accuracy than TOF [22].…”

“…Using histologic confirmation, numerous studies have shown that MRI exhibits high contrast for internal plaque features and that combined information from multiple contrast weightings is critical for distinguishing all plaque components [1116]. The purpose of this article is to summarize the current state of MRI of carotid atherosclerosis and to discuss the potential utility of MRI in clinical practice and research.…”

“…Biological processes, such as inflammation and neovascularization are capable of being identified by MRI. This not only could allow screening for high-risk patients, but it could also make it possible for early treatment initiation and noninvasive monitoring [33,34].…”
