High‐risk HPV positivity is a long‐term risk factor for recurrence after cervical excision procedure in women living with HIV

Abstract: Objective: To evaluate the risk factors for recurrence of high-grade disease after cervical excision in women living with HIV, with a specific interest in the role of HR-HPV positivity. Methods: Multicentric retrospective study conducted on WLWH who underwent cervical excision between January 1987 and June 2017 in six Italian institutions. The rate of highgrade recurrence was determined. Risk factors for recurrence and HR-HPV positivity were determined with the Log-rank test and Cox proportional-hazards regress… Show more

“…(iv) The present analysis did not consider several features that might influence patient outcomes. In particular, we were not able to correct our results on patient medical history, smoking history, and sexual activity [21][22][23]. All those points might influence the primary outcome measure and should be considered in everyday clinical practice.…”

“…All those points might influence the primary outcome measure and should be considered in everyday clinical practice. However, most of these features (indirectly) impacted the risk of HPV persistence, which is one of the variables that we included in our model [21][22][23][24][25]. (v) Our analysis excluded patients undergoing cold-knife conization.…”

“…Terlebih pada perempuan dengan HIV yang berusia lebih dari 40 tahun yang diperkirakan empat kali lebih berisiko untuk mengalami kekambuham. Secara keseluruhan kekambuhan pada perempuan dengan HIV mencapai 21,4% (Agarossi et al, 2021;Pambinkavil et al, 2023).…”

“…HPV variants are further classified into high-risk and low-risk categories, primarily determined by their association with the development of cancer [48]. High-risk HPVs, such as HPV16, HPV18, HPV31, and others, display a notable preference for mucosal tissues and are strongly correlated with various cancers, including cervical, anal, and oropharyngeal cancers [49]. These high-risk variants carry specific viral oncoproteins like E6 and E7, which disrupt the regulatory mechanisms of host cells, fostering uncontrolled cell growth and potentially progressing towards malignancy [50].…”
