Background Nigeria has a high burden of hepatitis B virus (HBV) infection, commonly acquired through vertical transmission. However, there is a lack of an efficient surveillance system for monitoring and understanding the epidemiology of HBV among pregnant women. Building on a previous review on the prevalence of HBV in Nigeria (2000–2013), we conducted a systematic review and meta-analysis of HBV prevalence among pregnant women in Nigeria. Methods Four electronic databases PubMed, Embase, Global Health, and Scopus were systematically searched from January 2014 to February 2021. We also searched the African Journal Online and manually scanned the reference lists of the identified studies for potentially eligible articles. Observational studies that reported the prevalence of HBsAg and/or HBeAg among pregnant women in peer-reviewed journals were included in the study. We performed a meta-analysis using a random-effects model. We defined HBV infection as a positive test to HBsAg. Results From the 158 studies identified, 20 studies with a total sample size of 26, 548 were included in the meta-analysis. The pooled prevalence of HBV infection among pregnant women across the studies was 6.49% (95% confidence interval [CI] = 4.75–8.46%; I2 = 96.7%, p = 0.001; n = 20). The prevalence of HBV was significantly lower among pregnant women with at least secondary education compared with those with no education or primary education (prevalence ratio = 0.7, 95% CI = 0.58–0.87; n = 10). However, the prevalence of HBV was not significantly different by age, religion, marital status, or tribe. The prevalence of HBV was not significantly different among pregnant women with previous surgery, blood transfusion, multiple lifetime sex partners, tribal marks, tattoos, scarification, or sexually transmitted infections, compared with those without these risk factors. From a total sample size of 128 (n = 7), the pooled prevalence of HBeAg among HBV-infected pregnant women was 14.59% (95% CI = 4.58–27.99%; I2 = 65.5%, p = 0.01). Subgroup analyses of HBV infection by study region and screening method, and meta-regression analysis of the study year, sample size, and quality rating were not statistically significant. Conclusions There is an intermediate endemicity of HBV infection among pregnant women in Nigeria. Interventions, such as routine antenatal HBV screening, antiviral prophylaxis for eligible pregnant women, and infant HBV vaccination should be scaled up for the prevention of perinatal transmission of HBV infection in Nigeria.

Background: Hepatitis B and C virus (HBV & HCV) infection is on the surge among pregnant women as a result of unsaved dental and cosmetic procedures, reuse of needles, raising the risk of perinatal transmission and HBV & HCV infection in children. Although few studies on the prevalence of HBV and HCV infection during pregnancy have been conducted but the majority of the studies are on very low sample size. The current study is based on large data set. Objectives: The major goal of this study was to find out the frequency of HBV & HCV in pregnant women. Methods: We conducted a retrospective cohort study in Alkhidmat Raazi Hospital Rawalpindi. A total of 31911 pregnant women who delivered between 2016 and 2021 were analyzed and categorized as HBV & HCV infected or HBV & HCV uninfected by billing codes. The birth certificate and hospital discharge records were used to identify infant records related to HBV and HCV-infected pregnant mothers. Pregnant women's plasma samples were tested for HBV and HCV using chemiluminescence ARCHITECT i1000SR® Abbott Diagnostics. Data were analyzed using packages within SPSS software and P ≤ 0.05 was considered significant. Cross-tabulation chi-square analysis was performed with a significance level of 5%. Results: The pregnant women with age under 25 years were 12699 (39.8%), women with age from 26 to 29 years were 9734 (30.5%), while women with age 30 and older were 9478 (29.7). The patients who were primigravida were 9514 (29.8%), multipara having 1-4 children 18545 (58.1%) and those having more than five children were 3852 (12.1%). The prevalence of HBV and HCV in 31911 pregnant women were 170/31911 (0.5%) and 478/31911 (1.5%), respectively. The mean age was 27.9 years. Conclusions: HBV and HCV infection is present in pregnant females, which may ultimately increase the treatment cost and vertical transmission. Hepatitis E is one of the most frequent causes of acute hepatitis worldwide, with an estimated 20 million infections and 70,000 deaths attributed to hepatitis E virus (HEV) genotypes 1 and 2 every year. This study was designed to determine the prevalence of HEV immunoglobulin M (IgM) in southwest (SW) Nigeria. Venous blood samples totalling 359 were collected from pregnant women on routine check, apparently healthy prospective blood donors and sick individuals presenting with fever and abdominal disturbance from health facilities in Ekiti, Lagos, Osun and Oyo states, of Nigeria. A structured questionnaire form was administered to gather socio-demographic data, health and travel history from each consenting participant. The screening for HEV IgM was done using HEV IgM ELISA kit. Statistical analyses, including descriptive analysis, correlations and binary logistic regression were carried out using SPSS version 21. In all, 131 samples (36.5%) tested positive for HEV IgM. Osun state had the highest occurrence of HEV IgM (n = 49; 13.6%,) while Ekiti had the least (n= 22; 6.1%). Apparently healthy participants with detectable HEV IgM were 52 (14.5%) while pregnant womenwere 51 (14.2%). Risk factors implicated in this study were flooding and injection drug use. The overall HEV prevalence reported in this study was higher than previous reports in Nigeria . Higher HEV prevalence in this study could be due to a larger coverage area in the country as well as detection of ongoing infection. The detection of HEV IgM in pregnant women and apparently healthy prospective blood donors indicated ongoing infection with risk of spread to susceptibles since HEV is neither routinely screened for among pregnant women nor among prospective blood donors.
