Hinged distraction of the hip joint in the treatment of Perthes disease

Abstract: The aim of this work is to determine the effect of this type of treatment on the shape of the femoral head, the range of motion (ROM), radiological changes in the femoral head, and the prognosis of Perthes disease at skeletal maturity. From 1998 to 2007, 53 patients with Perthes disease were treated with a combination of soft tissue release and joint distraction with a hinged monolateral external fixator in 32 patients and by Ilizarov external fixator in 21 patients. Nineteen of our 53 patients attained skelet… Show more

“…The advantages of this procedure are that it improves the range of motion, reduces superior and lateral subluxation, and provides better radiographic sphericity of the femoral head. This treatment can even be performed with distraction in stiff hips and deformed hips [ 1 , 2 , 21 , 27 ]. In a comparative study by Voplon in which he used arthrodistraction as a primary treatment for active forms of LCP disease and prospectively compared the results with those obtained using Salter innominate osteotomy, although the methods gave similar final radiological results, morbidity was higher with arthrodistraction than with innominate osteotomy.…”

“…27,28 The role of arthrodiastasis in LCPD has been evaluated in several studies. Most of these studies 4,14,17 performed soft tissue release during the index procedure. They all observed an improvement of the range of motion in the hip joint.…”

“…14,15 Different types of external fixators have been used in arthrodiastasis. [15][16][17] Permission of the joint motion through using an articulating device during arthrodiastasis allows synovial circulation and cartilage nourishment to occur, which further promotes healing of the necrotic head. 18 A paucity of data is available in literature that encourages the use of arthrodiastasis in the treatment of LCPD.…”

“…Since 1994 no other paper on arthrodiastasis and its applications in hip avascular phenomena in adults has been published. Nonetheless, papers reporting hip distraction in treating Legg-Calvé-Perthes (LCP) disease in children and adolescents have been published 13–15. Hip distraction is considered a salvage procedure in severe cases with late onset, where all other traditional methods offer poor outcomes.…”
