Hip arthroplasty in patients with cirrhosis of the liver

Abstract: e retrospectively reviewed 45 hip arthroplasties which were performed over a period of 20 years in 38 patients with cirrhosis of the liver. There was a high perioperative 30-day complication rate (26.7%). Advanced cirrhosis was associated with a higher risk of complications (p = 0.004) as also was increased age, a high level of creatinine, a low level of albumin, a low platelet count, ascites, encephalopathy and an increased operative blood loss. The survival of the prosthesis at five years was 77.8% and infec… Show more

“…14 The only previously study that exclusively evaluated the safety of hip arthroplasty is a recent uncontrolled study from Taiwan that overall perioperative complication rate was 26.7%. 17 The previously published data on the outcome of TKA in cirrhotic patients were included 41 Child's A cirrhotic patients and only 1 Child's B patient from a recent study in Taiwan. 11 They reported an overall perioperative complication rate was 43.1%.…”

“…In all three cirrhosis subtypes, the largest effect size was seen for thrombocytopenia. This may be due to the fact that thrombocytopenia is associated with chronic liver disease[29]; furthermore, thrombocytopenia increases the risk for complications following TJA[13,15,25]. …”

“…However, data are extremely limited on the safety and outcome of hip or knee arthroplasty in patients with advanced liver disease (12, 13, 19). Ziser et al (8) evaluated 733 patients with a diagnosis of liver cirrhosis who had undergone some type of surgical procedure, and reported a perioperative complication rate of 30.1% and a mortality rate of 11.6% within 30 days of surgery.…”

“…Only two recent studies (12, 13), to the best of our knowledge, have evaluated the perioperative risks of hip arthroplasty in cirrhotic patients. The purpose of this study was to document perioperative morbidity and mortality in patients with liver cirrhosis undergoing hip arthroplasty.…”
