Hip arthroscopy in osteoarthritis: Is it an option?

Abstract: Arthritis of the hip is a degenerative disease characterised by pain and inflammation. It is common and most often affects middle-aged to older adults, with the definitive management being total hip replacement. Advances in the surgical techniques has brought about the popularity of hip preservation surgery in patients with pre-arthritic hip abnormalities, with a goal to prevent progression to early arthritis and subsequently prolong the need for arthroplasty. There is a large body of evidence correlating femor… Show more

“…Older age and osteoarthritis have been established as predictors of both HA failure and adverse outcomes after conversion to THA. 12,15,18,63 The marked differences in our results compared with an analysis limited to older patients who underwent HA for osteoarthritis may support the conclusions made in previous studies regarding the limited utility of HA in patients with significant osteoarthritis. 15,18,44,63,66 Nonetheless, these data suggest that prior HA is not universally associated with increased rates of short-term prosthesisrelated complications after conversion to THA relative to age-, sex-, and comorbidity-matched controls without prior HA.…”

“…It suggests that the chronic mechanical stress induced by the FAI mechanism, accelerates the labral deterioration and contributes to labral tears and symptoms onset. FAI also leads to chondropathies that cause pain and limited functional activities affecting activities of daily living ending with OA [ 32 ]. Moreover, the worst labral degeneration, with ECM loss, strong prevalence of proteoglycans, and large calcium deposits was observed in the older FAI patient.…”

“…[2][3][4] Hip surgery, including hip arthroplasty, hip fracture internal fixation and hip arthroscopy procedures, is the main treatments for hip diseases. [5][6][7][8] Hip surgery is often associated with moderate to severe postoperative pain, particularly in hip fracture patients undergoing surgical treatment and severe pain persists throughout the perioperative period. [9][10][11] As a minimally invasive approach, arthroscopic hip surgery is gaining popularity globally.…”
