Hip arthroscopy learning curve: a prospective single-surgeon study

Abstract: Hip arthroscopy provides very good clinical outcomes if precisely indicated and performed. It is, however, a demanding procedure with many possible pitfalls and complications. According to our study, at least 100 procedures are needed to gain basic technical and indication skills. The presence of a more skilled surgeon in the beginning of the learning curve is advised to reduce the complication rate.

“…Recognition of the difficulties of hip arthroscopy and its potential complications 15,16,19 has generated a flurry of studies of the associated learning curve. 4,6,7,9,17,19 Most of these publications have reported the experience of a single surgeon, 6,7,9,17 with outcome metrics that include operative time, 6,7,9 reoperation, 7 fluoroscopy time, 17 complications, 6,7 conversion to total hip arthroplasty, 6,7 and patient-reported outcome measures. 6,7,9 While single-surgeon reports are edifying, they reflect the abilities and experience of the reporting surgeon, as well as his or her surgical team, facility, procedure mix, and patient characteristics, and thus may not be generalizable to other practitioners.…”

“…Vastus lateralis and glut max transfers are mostly indicated in patients with chronic glut tears and associated muscle atrophy 18 . Endoscopic repairs require extensive arthroscopic training and have limitations in visualization and capacity of reducing the tendon to the footprint in chronic cases with severe adhesions 19 . Reconstruction with the Achilles tendon allografts is expensive and not readily available in centers without appropriate human tissue licensing arrangements in place 8 …”

“…Reagan et al (Reagan, McInerny et al, 1983) 8 compared 35 SO 4 uptake by bovine articular cartilage specimens and found that the uptake was higher with Ringer's lactate compared to normal saline. They felt that the acidic pH (5.3) of the saline solution used in their study could be responsible for this difference in uptake.…”
