Summary Objectives This article presents the 2009 selection of the best papers in the special section dedicated to biomedical informatics and cybernetics. Method Synopsis of the articles selected for the IMIA yearbook 2009 Results Five papers from international peer reviewed journals where selected for this section. Most of the papers have a strong practical orientation in clinical care. And this selection gives a good overview of what is done with “closing loop” approach, particularly during theyear 2008. Conclusion While quite mature for some clinical applications such as mechanical ventilation, it remains a challenge where rules for the decision system could be difficult to identify due to the number of variables. More complex systems with greater Artificial Intelligence approaches will certainly be the next trend for closed-loop applications.

This study aims at exploring the role of user innovativeness in the home telehealth acceptance model, clarifying whether innovativeness is the antecedent to or moderator of the Technology Acceptance Model (TAM); it also aims to discover the influences of related discussion on user acceptance. The results show that innovation is both the antecedent to and the moderator of TAM. Attitude toward using home telehealth services could be enhanced through improvement of individual innovation. Suggestions are proposed for individuals with high or low innovation, respectively.

Mountain regions are important producers of organic food. For them, reliable decision making regarding business planning necessitates different critical support methods. KARSIM 1.0 (DSM) is a methodology based on an integrated deterministic simulation system application for decision-making support, consisting of 74 deterministic production simulation models. DSM enables different types of cost and financial feasibility calculations for organic production and food processing. KARSIM 1.0 was used to simulate three specific business alternatives for mountain organic farms. (Alternative 1: spelt grain, fruit cider, wine and brandy, plum brandy, calves meat and sheep-soft cheese production, Alternative 2: spelt flour, pear and apple juice, plum brandy, veal and sheep's milk, Alternative 3: spelt grain, dried fruit, calves, soft and hard sheep cheese). Simulation model results were compared using two multi-objective analysis methods: the analytical hierarchical process (Expert Choice Decision Support System software) and DEX-i method. The results showed the highest multi-objective decision evaluation for alternative 2 (Expert Choice = 0.361 and DEX-i evaluation = excellent). We can conclude that the combination of a deterministic cost simulation model and multi-criteria decision analysis presents an acceptable decision support tool for mountain organic farms; however, further research is desirable.
