Background: Influenza causes high mortality rate among pregnant women, while morbidity and mortality cases of this disease and its side effects among pregnant women can be simply prevented by vaccination and reducing risk factors such as cardiovascular diseases, diabetes, hypertension, etc. Objectives: Hence, this study was conducted to investigate the risk factors and mortality rate of seasonal influenza among pregnant women referring to healthcare centers in Iran during 2015 -2016. Methods: This case-control study was conducted to examine the odds ratio of seasonal influenza among pregnant women with seasonal influenza who had referred to all healthcare centers in Iran with severe respiratory disease symptoms during 2015-2016. The statistical population consisted of 2,080 pregnant women of whom, 24 dead women constituted the case group and 100 randomly selected living women constituted the control group. A researcher-made checklist was used for data collection. The collected data were analyzed using the statistical test of the odds ratio with SPSS-22 software. Results: Mean (standard deviation) of age was 31 (2.3) and 34 (3.8) in living and dead pregnant women, respectively. The most frequent type of conflicting influenza virus was H1N1 in both case and control groups (83% and 71%, respectively). There was a significant difference between case and control groups in the risk factors including diabetes (P = 0.003), blood pressure (P = 0.001), obesity (P = 0.026), cardiovascular diseases (P = 0.001), renal diseases (P = 0.013), and respiratory diseases (P = 0.012). Among these risk factors, the highest odds ratio (OR) was related to cardiovascular diseases (OR = 24), blood pressure (OR = 16.3), and diabetes (OR = 12.9), in sequence. Oseltamivir prescribed to all patients in the control group and 92% of patients in the case group. Seven patients in the control group and none in the case group had a history of influenza vaccination. Conclusions: It is necessary to take steps and underpin training programs to reduce the risk factors of seasonal influenza among pregnant women based on the national vaccination guidelines.

Introduction: The surgical procedure known as one-step conservative surgery for placenta accreta spectrum involves the excision of the placenta and its attachment site on the myometrium. We aimed to conduct a non-randomized comparative study between conservative surgical treatment of placenta accreta spectrum and classical caesarean hysterectomy, under the name of the "CMNT PAS" study. To determine the sample size and the expected difference between the two groups, we conduct this pilot study. Study design: It was a monocentric prospective observational study from January 4, 2020, to August 17, 2022. Patients were allocated into two groups: Group Conservative Surgery (CSG) in which the method detailed in prior research by Palacios-Jaraquemada was slightly modified by our team, control group (CG) when the caesarean hysterectomy was performed. Results: Our main goal is to determine the sample size for our principal study and provide adequate data for any other team wishing to study the conservative surgical treatment of placenta accreta spectrum. So, the sample size for a power of 90% and an alpha risk of 5%, the size of each group must be 22 patients. Regarding primary outcomes: estimated blood loss volume was lower in CG compared to CSG but not statistically significant. Conclusion: Our pilot study found that the caesarean hysterectomy is similar to conservative surgical treatment for placenta accreta in estimated blood loss. However, the latter helps preserve uterus and possibly fertility.

Postpartum hemorrhage (PPH) is the most cause of maternal mortality all over the world important. Its prevalence is about 4 cases in 10,000 deliveries. However, the risk of maternal death is nearly 16% of all maternal deaths in developing countries, especially in Africa. It depends on the resources to stop bleeding, medical and surgical. The first ones are uterotonics medication (prostaglandin, methylergonovine and oxytocin), and the latest professional apt to perform surgical procedures, such as B-Lynch surgery and the peripartum hysterectomy. A woman, 20 years old, GI, P0, A0, with a single pregnancy, 42 weeks of gestational age and 5 prenatal bookings was attended at the emergency on April 12 th 2015. The next day the patient delivered a male baby. The uterus was soft and at the examination she expelled a great amount of clots. She was prescribed Ringer lactate 500 ml + oxytocin 15 IU + methylergonovine 0.2 mg IM + misoprostol 800 µg per rectum. She continued bleeding and a hysterectomy was performed. The patient was discharge in good condition. The pathology revealed myometrium permeated by trophoblasts (placenta accreta).
