Histologic Analysis of Endoscopic Ultrasound-Guided Through the Needle Microforceps Biopsies Accurately Identifies Mucinous Pancreas Cysts

Abstract: In a multicenter prospective study of patients undergoing EUS-FNA for evaluation of PCLs, we found TTNB collection of tissues for histologic analysis to be safe and feasible, with an acquisition yield of 83.3%. Histologic analysis of samples collected by TTNB identified a larger proportion of mucinous PCLs compared with cytologic analysis of samples collected by FNAeven among samples categorized as equivocal, based on the level of carcinoembryonic antigen. More samples collected by TTNB than FNA were found to … Show more

“…Nevertheless, this would probably not have affected the results because the patients included in our cohort were sent for EUS-FNA after inconclusive CT and MRI. (3) We did not include in our study new PCLs diagnostic tools for which the potential interests have been published during the conduct of the study, including DNA mutations [25,26], string sign tests [27], and EUS-guided microforceps biopsies [28]. (4) For the sake of simplification, therapeutic management was based on a consensus of a panel of five experts.…”

“…In all cases, bleeding was self-limiting and asymptomatic and did not require medical intervention. Three moderate AEs were recorded, two by Kovacevic et al [17] and one by Yang et al [20]. All were mild acute pancreatitis.…”

“…All 3 cases of moderate adverse events were mild acute pancreatitis. In the study of Yang et al [20], one patient developed a pseudocyst several weeks later that required endoscopic drainage, the patient recovered after stent removal without recurrence.…”

“…Sensitivities for diagnosing mucinous cyst and malignant transformation using cyst fluid obtained by EUS‐FNA were not particularly high. Recently, EUS‐guided through‐the‐needle microforces biopsy was reported to improve the pathological yields and showed better diagnostic potential than that with EUS‐FNA . Further evaluation of this technique is required, as diagnostic yields might be limited considering possible patchy distribution of specific pathological findings for differential diagnosis on the cyst wall.…”
