Histopathologic Characterization of Myocarditis Associated With Immune Checkpoint Inhibitor Therapy

Abstract: Context.— Cardiac complications of immune checkpoint inhibitor therapy are rare, but reports of myocarditis are increasing. The findings have been described in case reports as lymphocytic myocarditis, but its histopathology is underreported. Objective.— To review the histology of myocardial biopsy–proven cases of immune checkpoint–associated myocarditis and provide immunohistochemical characterization of the inflammatory infiltrate. Design.— We have encountered 6 patients with biopsy-proven myocarditis in conj… Show more

“…CD8 + T cells are the effectors of ICI anti-tumour effects, and high levels of CD8 + T cell infiltration in tumours before and during ICI treatment are associated with improved ICI efficacy [ 23 ]. The extent of CD8 + T cell mucosal infiltration in CIC reported in this study is comparable to heavy CD8 + T cell infiltration previously described in non-intestinal sites of autoimmune inflammation [ 24 27 ].…”

“…observed the same diffuse lymphohistiocytic myocarditis in 6 patients by EMB. The infiltrates were characterized by a population of PD-1–positive, CD8-positive, granzyme-B–positive T lymphocytes ( 18 ). They further found the immunoreactivity for PD-L1 in damaged area and proposed that the expression of PD-L1 on myocytes might be the key factor for avoiding self-targeting immune responses, which was in line with the second probable mechanism.…”

“…Recent reports indicate that the relatively distinctive histologic feature of ICI-induced myocarditis is diffuse lymphocytic infiltration. This conclusion needs to be further verified due to the low number of endomyocardial biopsy (40).…”
