History of cervical disc arthroplasty

Abstract: Enthusiasm for cervical disc arthroplasty is based on the premise that motion-preserving devices attenuate the progression of adjacent-segment disease (ASD) in the cervical spine. Arthrodesis, on the other hand, results in abnormal load transfer on adjacent segments, leading to the acceleration of ASD. It has taken several decades of pioneering work to produce clinically relevant devices that mimic the kinematics of the intervertebral disc. The goal of this work is to trace the origins of cervical arth… Show more

“…By preserving motion N.A. White et al 306 and loading distribution, some consider CTDRs a method to reduce the likelihood of ASD (Galbusera et al 2008;Baaj et al 2009;Bartels et al 2010;Fekete and Porchet 2010;Kishen and Diwan 2010;Uschold et al 2011). The use of cross sections to measure load contributions from the cervical zygapophysial and uncovertebral joints will also be investigated in future studies.…”

“…Early in the decade this effort was largely aimed at lumbar arthroplasty. 13 The Cummins-Bristol cervical disc was the first such attempt. 10 Other manufacturers followed suit, and this resurgence set the stage for new attempts at cervical arthroplasty.…”

“…10 The SB Charité (DePuy Spine Inc., Raynham, MA) was designed in 1982, implanted in 1984, commercially available in 1987, and ultimately FDA approved in 2004. 13 Results were published in 1998 for 22 Cummins devices implanted in 20 patients. 13 The Cummins-Bristol cervical disc was the first such attempt.…”

“…Cervical disc arthroplasty devices have been developed as an alternative to fusion techniques, with the aim of preserving motion and diminishing the rate of ASD acceleration 1. However, little is known as to whether Total Disc Replacement (TDR) is also effective in the treatment of the degenerated disc adjacent to a prior fusion, especially in patients with Klippel-Feil syndrome (KFS) who have “cod-fish” narrow vertebrae with resulting small endplates.…”
