HIV drug resistance in persons who inject drugs enrolled in an HIV prevention trial in Indonesia, Ukraine, and Vietnam: HPTN 074

Abstract: BackgroundPersons who inject drugs (PWID) have high HIV incidence and prevalence, and may have limited access to antiretroviral therapy (ART) in some settings. We evaluated HIV drug resistance in PWID in a randomized clinical trial (HPTN 074). The study intervention included ART at any CD4 cell count with enhanced support for ART and substance use treatment.MethodsHPTN 074 enrolled HIV-infected PWID (index participants) with viral loads ≥1,000 copies/mL and their HIV-uninfected injection-network partners in In… Show more

“…A total of about 35,000 Vietnamese immigrants were estimated to be in Bulgaria from 1980 to 1991, but most returned to Vietnam in 1991 after Bulgaria became an independent democracy in 1991. Combined, these results and historical information suggest a possible introduction of CRF01_AE into Bulgaria from Vietnam immigrants between 1988 and 1990 with subsequent rapid transmission via PWIDs, which is similar to other recent global outbreaks [ 35 , 36 , 45 , 46 , 47 , 48 ].…”

“…In recent years, the impact of HIVDR on treatment outcomes, particularly among pediatric and adolescent populations [ 24 , 25 ••, 26 , 57 ], men who have sex with men (MSM) [ 58 ], and injection drug users [ 59 ], has become a growing concern. Alarming rates of HIVDR to NNRTIs have been reported in association with expanded access to ART, PMTCT, and prolonged use of failing ART regimens in patients without virologic suppression [ 24 , 46 , 50 ••, 60 ].…”

“…Drug resistance was detected in nearly 90% of the FSW who had ARV drugs detected. This is higher than the proportion that has been observed in other cohorts and settings [ 22 , 23 , 48 ]. Further studies are underway using quantitative ARV assays to evaluate whether HIV drug resistance among FSW is associated with sub-optimal ART adherence.…”
