HIV incidence increasing in MSM in Germany: factors influencing infection dynamics

Abstract: After an initial peak in the mid-1980s, HIV incidence in men who have sex with men (MSM) declined in most western industrialised countries and then levelled off during the 1990s. Since the late 1990s, increasing numbers of newly diagnosed HIV infections in MSM have been observed in the majority of countries with large and visible MSM communities. Based on a review of national and international behavioural surveillance studies of MSM and national HIV surveillance data, we propose a model for the HIV epidemic in… Show more

“…29 In the same period, in the United Kingdom, Germany, and Italy, a significant increase in the incidence of HIV-1 infection in cohorts of MSM was also observed. [30][31][32] Also in France among the MSM with a new HIV-1 diagnosis the highest proportion of recent infections (46.0%) and an elevated percentage of non-B subtypes (12.0%), after 2003, have been observed. 19 Nevertheless, there are relatively few data on HIV-1 genetic diversity among MSM in Europe, and most of the available data have been provided by national surveillance systems 33,34 or from serosurveys of individuals attending HIV-1 screening programs in urban STI clinics.…”

“…It is evident that the differentiation between steady and casual partners leads to different distribution of sexual practice. According to the literature, in some countries the number of sexual partners seems to have increased in recent years [25,26]. This may be a good proxy variable for unsafe sex [9].…”

“…13,14 Second, 'seropositioning', where HIV-negative MSM adopt the insertive role in UAI to reduce their HIV risk, is being commonly reported throughout the world. 5,15 Should increasing proportions of uncircumcised HIV-negative MSM take the insertive role in UAI with HIV-positive partners, then the proportion of new HIV infections attributable to insertive UAI is likely to increase.…”

“…[2][3][4][5] Much research in recent years has focussed on potential biomedical interventions to prevent HIV transmission. Strategies, including candidate microbicides and vaccines, have thus far failed to provide protection against HIV infection.…”
