HIV treatment is associated with a twofold higher probability of raised triglycerides: pooled analyses in 21 023 individuals in sub-Saharan Africa

Abstract: Background Anti-retroviral therapy (ART) regimes for HIV are associated with raised levels of circulating triglycerides (TGs) in western populations. However, there are limited data on the impact of ART on cardiometabolic risk in subSaharan African (SSA) populations. Methods Pooled analyses of 14 studies comprising 21 023 individuals, on whom relevant cardiometabolic risk factors (including TG), HIV and ART status were assessed between 2003 and 2014, in SSA. The association between ART and raised TG (>2.3 mmol/… Show more

“…However, ART is now initiated most often in people living in sub Saharan Africa, where data defining the metabolic effects of treatment are scarce. In a 2018 meta analysis of 14 trials and including a total of 21,023 individuals from this region, ART was associated with an increased risk of hypertriglyceri daemia (RR 2.05, 95% CI 1.51-2.77); however, no con sistent associations were observed between ART and raised blood pressure, blood glucose levels, glycated haemoglobin (HbA 1c ) levels or other blood lipids 135 .…”

“…In a recent meta-analysis of 14 trials of 21,023 individuals assessed between 2003 and 2014 from this region, ART was associated with an increased risk of hypertriglyceridemia (relative risk, 2.05; 95% confidence interval [CI], 1.51-2.77). 26 The associations between ART and raised blood pressure, glucose, hemoglobin A1c, and other lipids were inconsistent across studies.…”

“…The antioxidative properties of HDL-C are also impaired because of a loss of antioxidant proteins and accumulation of oxidized phospholipids. 26 Macrophage myeloperoxidase, which is induced by inflammatory stimuli in atherosclerotic plaques, also mediates reductions in HDL-C function via oxidation. “…The prevalence of hypertension in PLHIV is estimated to range from 9 to 46% in low-and middle-income countries (LMICs) [13,14], similar to high-income countries [15][16][17]. Patients on ART are more likely to have dysglycemia, hypertriglyceridemia, and lower levels of high-density lipoprotein cholesterol [18][19][20], which may increase hypertension risk. Also relevant, studies indicate that HIV may increase the risk of non-communicable disease (NCD), including cardiovascular disease CVD, due to stimulation of inflammatory markers and adverse events of ART [21][22][23][24].…”
